MASHPEE HIGH SCHOOL BID EVALUATION FORM MASHPEE, MASSACHUSETTS EQUIPMENTS BIDS - 2/8/96 PAGE 6 of 6 SECTION 102 - OFFICE FURNITURE AND EQUIPMENT NOTES-BID QUALIFICATIONS: ROBERT H.LORD COMPANY: 1.) Delivery by August 1996, if order is received by March 1996. See cover letter attached to bid for individual manufacturers' delivery lead times. 2.) All items bid in manufacturers' standard colors and construction. 3.) Item OF-17, Allsteel Staff Cubicle, is bid with pedestals lock only; center drawer does not lock. 4.) In the case of any construction delay, R. Lord Co. requires 30-days advance notice prior to requested ship date in order to change delivery date. 5.) Should a delay warrant storage charges, the fee will be $500.00 per month with a one-month minimum. 6.) Re-delivery charage from the storage facility to the site will be billed at $25.00/man, per hour. 7.) If items are to be off-loaded at the site and put into storage to be installed/assembled at a later date, the Town will be charged for the extra trip at a rate of $25.00/man hour, including travel time. SHAMROCK DISTRIBUTING: 1.) Delivery, as requested. OFFICE RESOURCES: 1.) Delivery, as requested. 2.) Bid pricing assumes use of elevators for delivery. Carry-up charges would be additional $15.00 per piece to carry up-stairs. BUSINESS FURNITURE: 1.) Delivery, as requested. MONITOR EQUIPMENT COMPANY: 1.) Delivery is 60-90 days ARO. MASS. CORRECTIONAL INDUSTRIES: 1.) Bid did not include a bid surety. SCHOOL SPECIALTY: 1.) Delivery is approximately 3-7 weeks ARO. 2.) Pricing is guaranteed for 60 days. 3.) All Krueger items bid in their standard manufacturer's colors. EQUIPMENT ENVIRONMENTS: 1.) Delivery is 45-75 days ARO/colors. SCHOOL FURNISHINGS: 1.) Delivery, as requested. Lead time is 8 weeks ARO. OFFICE FURNITURE DISTRIBUTORS: 1.) Delivery, as requested. 2.) All delivered and installed unit price/total price is based on regular working hours (Mon.-Fri.) under normal working conditions (i.e. worksite free and clear, exclusive use of loading dock and elevator) using non-union labor. 3.) Delivered and installed pricing is also based on product being shipped from the manufacturers direct to the school. There will be additional charges if the product has to be offloaded and stored at an alternate site and then re-delivered at a later date. SLAYCO, INC.: 1.) Pricing includes delivery and installation; pricing will hold for 90 days. 2.) All Haskell items are bid as specified in manufacturer's standard colors. 3.) All Krueger items are bid as specified in manufacturer's standard colors.
