MINUTES May 4, 2000 3 BOARD OF HEALTH <br /> Discussion took place regarding the possibility of this lot accommodating a Title V <br /> septic system. Board members agreed to allow the applicant to conduct a de-watered <br /> perc test on the lot. No further action will be taken by the Board of Health. Members <br /> agreed that plans must be submitted to the Board of Health showing a Title V septic <br /> system with no variances. <br /> 2. Letter from "Contrast" re: outdoor seating <br /> Board acknowledged. <br /> 3. Fax from Gary Eynation re: 55 Uncle Edwards Road <br /> Mr. Ed Govoni of Dream Developers was in attendance for this agenda item. <br /> Mr. Eynation submitted a letter requesting the Board review the location of the <br /> septic tank in relation to the well (location unknown) at 55 Uncle Edwards <br /> Road. <br /> Mr. Govoni submitted revised plans showing the new location of the septic tank. <br /> Mr. Doherty motioned to have the agent respond to Mr. Eynation regarding his <br /> letter and the revised plans. Mr. Cram seconded the motion. Motion passed. <br /> 4. Response from C. Costa re: 20 Lady Slipper Lane <br /> Board acknowledged. <br /> 5. Perc extension request- 123 Rock Landing Road <br /> Applicant is requesting a 6-month extension for the perc tests done at 123 Rock <br /> Landing Road. <br /> Mr. Cram motioned to grant a 6-month perc test extension for 123 Rock <br /> Landing Road. Mr. Doherty seconded the motion. Motion passed. <br /> 6. Emergency Failure Certifications: <br /> 20 Wampanoag Drive <br /> Mr. McQuaid indicated that the liquid in the tank is above the outlet, therefore <br /> the pit failed. New distribution box and 1000-gallon pit need to be installed. <br /> Board members forwarded their signatures to the Emergency Failure <br /> Certification form. <br /> NEW BUSINESS <br /> a. Moe's request to replace tire trailer <br /> Mr. Cram motioned to direct the Health Agents to research the need for a new <br /> tire trailer at the transfer station. Mr. Doherty seconded the motion. Motion <br /> passed. <br /> b. Deer Crossing Market & Dino's <br /> Mr. McQuaid presented Board members with a letter from the DEP indicating <br /> their approval for the reduced flow calculation for these two restaurants based <br />
