Request for Sticker - Summerwood Condominiums <br /> Summerwood Condominiums is requesting a transfer station sticker for their <br /> maintenance supervisor to be able to dispose of items that are not appropriate <br /> to be put in the dumpster. <br /> Mr. Ball stated that the condominium association could purchase a punchcard <br /> for 10 trips to the transfer station for $50.00. <br /> Mr. Doherty motioned to deny the request. Mr. Santos seconded the motion. <br /> Motion to deny passed. <br /> 7. Letter from R. Taylor re: Indian Museum <br /> The architect for the Mashpee Wampanoag Indian Museum is asking for the <br /> Board of Health to sign off on a building permit for the renovation of the <br /> museum. However, there is only a cesspool servicing the museum at this time. <br /> They are proposing to build a separate restroom building at the site which <br /> would require a Title V upgrade. Mr. McQuaid stated the Board's requirements <br /> in a letter to Maurice Foxx of the Wampanoag Tribal Council dated April 28, <br /> 2000. <br /> Mr. Doherty motioned to direct the Health Agent to respond to the request by <br /> Taylor MacDougall Burns Architects and reiterate Mr. McQuaid's letter and ask <br /> them to come before the Board for further clarification if necessary. Mr. Santos <br /> seconded the motion. Motion passed. <br /> 8. Variance request: 34 Pondview Drive <br /> The applicant is requesting a 3.10'variance from the septic system to the street <br /> line at 34 Pondview Drive. <br /> Mr. Santos motioned to grant the variance with a stipulation that the Board <br /> receives a copy of the service contract and monitoring contract for the <br /> denitrification system for general use prior to occupancy. Mr. Doherty <br /> seconded the motion. Motion passed. <br /> 9. Emergency Failure Certification <br /> 36 Lakewood Drive <br /> Mr. McQuaid stated that the leach pit is full. The installation of 3 drywells and <br /> a distribution box with 3'of stone is necessary (4 existing bedrooms). <br /> Board members forwarded their signatures to the Emergency Failure <br /> Certification form. <br /> NEW BUSINESS (cont'd) <br /> Letter to Ed Eichner of the Cape Cod Commission <br /> Board acknowledged. <br />
