MINUTES June 1, 2000 2 BOARD OF HEALTH <br /> Mr. Santos motioned to approve the food service establishment permit request <br /> by Michelle's Kitchen and require the $125 permit fee. Mr. Doherty seconded <br /> the motion. Motion passed. <br /> 6. Variance Request <br /> 156 Waterway <br /> Applicant is requesting a variance of 13'from the septic tank to the foundation <br /> and a variance of 4'from the soil absorption system and reserve area to the <br /> street property line. <br /> Mr. Ball noted that the variances as requested in the cover letter were <br /> inaccurate and needed to be changed. He also stated that he would require the <br /> inside and outside of the foundation to be sealed. <br /> Mr. McQuaid also noted that the application needed to be changed to indicate <br /> three bedrooms. <br /> Mr. Santos motioned to grant the variances requested with a change in the <br /> cover letter, and the change of bedrooms on the application. Mr. Doherty <br /> seconded the motion. Motion passed. <br /> 12 Slice Way <br /> Applicant is requesting a variance of 8' from the street property line to the <br /> active and reserve systems. <br /> Mr. Doherty motioned to grant the variance of 8' from the street property line to <br /> the active and reserve systems pending revised cover letter showing the correct <br /> variance request. Mr. Santos seconded the motion. Motion passed. <br /> NEW BUSINESS <br /> a. Emergency Failure Certification: 359 Cotuit Road <br /> Mr. McQuaid indicated that the leach pit was overflowing onto the ground <br /> surface. The installation of two leaching chambers and a distribution box with <br /> 3' stone is necessary. <br /> Board members forwarded their signatures to the Emergency Failure <br /> Certification form. <br /> APPOINTMENT• Cape Cod Commission Life at Mashpee SouthCape Village <br /> In attendance were: Michael Bliss of Connors & Bliss, P.C. on behalf of Talanian <br /> Realty and SouthCape Village, Ed Eichner of the Cape Cod Commission, Don <br /> Megathlin of Talanian Realty, Dennis Mahoney and Marianne Matthews of Life at <br /> Mashpee, and Charles Talanian of Talanian Realty. <br /> Mr. Bliss stated, "I specialize in the area of land use and environmental law. I've had a <br /> lot of experience in this area. I'm here on behalf of Talanian and the SouthCape <br /> Village project. Our purpose in meeting with the Board tonight is to clarify, hopefully <br /> and finally, the entitlement of Talanian to the nitrogen removal credit that they have <br />
