SECTION VI <br /> HEALTH (School Nurse) <br /> The Nurse is on duty during the entire school day. Students who feel ill in the school must <br /> have authorization from staff to visit the Nurse's Office. <br /> Absence - When a child is absent, it is the responsibility of the parent or guardian to <br /> telephone 508-539-6708 or 508-539-1520, extension 104 by 9:30am to inform the school <br /> that the child will not be attending that day. <br /> In addition to phoning in an absence,parents/guardians must send in an absentee note with <br /> the child upon returning to school after an absence. <br /> Emergency Card—The emergency card needs to be updated immediately if you have any <br /> changes in your employment, home phone number, home address, cell phone number or <br /> emergency contact persons (two local individuals). <br /> Be aware that your emergency contact person will be contacted and responsible for your <br /> child in the event of an injury or illness if we are not able to reach you. If the individual <br /> who comes to school to pick up your child is not on the emergency card, but has been <br /> designated by you for pickup, he/she will be required to produce a picture ID upon <br /> arrival to school. If no one can be reached in the event of an emergency, 911 will be <br /> called, and your child will be transported to the nearest hospital. <br /> Dismissal for illness - The Nurse will determine whether the student is not well enough to <br /> remain in school and the parent/guardian will be contacted. Parents/guardians are required to <br /> list two alternate names on the Emergency Procedure Cards of persons to be contacted if the <br /> parent/guardian is unavailable. <br /> Immunization - It is the parent's/guardian's responsibility to provide immunization and <br /> subsequent booster information at the time of registering a child in school. When a student <br /> does not meet full immunization/physical exam requirement according to the Massachusetts <br /> Department of Public Health and Massachusetts General Laws, Chapter 76, Section 15, the <br /> following steps s will be taken: <br /> • Parents/guardians will receive a phone call from school personnel regarding the missing <br /> required information. <br /> • If updated records are not received or notification of a medical appointment within one <br /> week written notification of missing immunization/physical exam will be sent to the <br /> parent/guardian with an exclusion date within three weeks <br /> • The student will be excluded from school by the date listed on the written notification if <br /> there is no updated immunization record/physical exam or documented medical <br /> appointment. <br /> Proof of immunizations for Massachusetts Public School students is required for diphtheria, <br /> pertussis, tetanus,polio, measles, mumps and rubella.
