Lead Poisoning—Parents/guardians must present documented evidence of lead poisoning <br /> screening prior to entrance into kindergarten (105 CHR 460). <br /> Physical Examinations—Required for students in grades kindergarten, 4, 7 & 10. It is <br /> recommended that a private physician or health care facility perform and document this <br /> examination. <br /> Physical Education Excuses - Students may be excused from physical education classes <br /> for medical reasons only. Students to be excused must bring a note, dated and signed from <br /> a physician stating limitation/restrictions and duration. <br /> Medical History/Needs -Parents/guardians are required to notify the school nurse at the <br /> start of or during the school year if their child has any medical conditions that may need <br /> to be monitored during the school year. This includes all food allergies, seizures, cardiac <br /> conditions, diabetes,migraines, asthma, bee sting allergies, etc. Please also notify the <br /> School Nurse if your child is taking medications at home but not at school. <br /> Confidentiality will be maintained. It is vital that the school be aware of medical <br /> conditions to ensure the safety of your child. <br /> Diagnosis/Treatment- School personnel is not responsible for diagnosis/treatment of <br /> students. School policy does not permit school personnel to diagnose/treat or to prescribe <br /> treatment. Emergency care is limited to first aid; the immediate and temporary care given <br /> in case of accident or sudden illness to save life or prevent further injury. Should a severe <br /> emergency develop and we are unable to reach family or emergency contacts, we will <br /> notify the local rescue service and the student will be transported to the nearest <br /> hospital. <br /> Inspections—Students may be inspected, as necessary, for pediculosis. If this condition is <br /> found, the student will be excluded until all lice and nits are removed. Students must be <br /> examined by the school nurse prior to returning to the classroom. <br /> Injury—Every accidental injury in school or on the grounds,no matter how slight, should be <br /> reported immediately to the staff member in charge and to the nurse. <br /> Counseling—The school nurse is available for health counseling. Students, parents/guardians <br /> are encouraged to discuss any health-related issues with the nurse. <br /> Screening Programs—Vision, hearing, height and weight screenings are performed <br /> annually on all students. Parents/guardians will be notified if there are any abnormal <br /> findings. A referral will be generated for a follow-up with a specialist. <br /> Please refer to the schedule of health services in this section. <br /> 50 <br />
