2. The permitting and installation shall be in accordance with Standard Conditions for Secondary <br /> Treatment Units Certified for General Use, last revised- March 20, 2015. <br /> 3. Add a vent to the Soil Absorption System near the driveway. <br /> 4. The alarm panel is to.be placed on the exterior of the dwelling for service. <br /> Laurel Almquist seconded the motion.The motion was unanimously approved. <br /> OLD BUSINESS <br /> 1. Sanitary Code Violations and Fines: 4 Tri-Town Circle. <br /> Attorney Brian Wall, representing Richard Halpern, owner of 4 Tri-Town Circle, introduced himself to the <br /> Board. <br /> Attorney Wall commented that Matt Costa of Cape and Islands Engineering provided Mr. Halpern with two <br /> proposals for the Nitrogen Aggregation Loading Plan. Mr. Halpern reviewed the information and decided with <br /> the 3-bedroom conventional Title 5 system. Mr. Costa informed Attorney Wall that the plan will be completed <br /> next week. <br /> Glen Harrington stated that the perc test was performed today. Poor soil and high groundwater were <br /> encountered, which was expected because of the proximity to the wetlands. Therefore, a Denitrification <br /> System with Ultra Violet disinfection may be required. <br /> Mallory Langler made a motion that the Board of Health continue the hearing on 4 Tri-Town Circle to the next <br /> Board of Health meeting scheduled on January 4, 2018. Laurel Almquist seconded the motion. The motion was <br /> unanimously approved. <br /> NEW BUSINESS <br /> 1. Sign Expense Warrants. The Board members signed the expense warrants. <br /> 2. Review/Approve BOH Public Session Minutes: November 30, 2017. <br /> Laurel Almquist made a motion that the Board of Health approve the Public Session minutes of November 30, <br /> 2017, as amended. Mallory Langler seconded the motion-The motion was unanimously.approved. <br /> 3. Request Fee Waiver for Temporary Food Events: SpinCloud Organic Cotton Candy. <br /> Mr. Harrington stated that Laurin Balfour has requested she be able to sell her product at temporary food <br /> events for one fee and not pay the daily temporary food fee. The Board had previously approved an annual fee <br /> to sell her food at temporary events for $100.00, which is the same as the mobile food permits. <br /> Mallory Langler made a motion that the Board of Health approve the fee waiver request for SpinCloud Organic <br /> Cotton Candy. Laurel Almquist seconded the motion. The motion was unanimously approved. <br /> ADDITIONAL TOPICS <br /> 1. Housing Condemnation: 117 Sunset Strip. <br /> Mallory Langler made a motion that the Board of Health add 117 Sunset Strip to the agenda. Laurel Almquist <br /> seconded the motion. The motion was unanimously approved. <br /> On December 7, 2017, Glen Harrington forwarded a copy of the inspection form, the determination of danger <br /> letter, and the affidavit for the search warrant to the Board for notification and for review by counsel. The <br /> dwelling had been placarded by the Assistant Health Agent, and once Mr. Harrington obtained the warrant he <br /> met the pest control people at 117 Sunset Strip. Ninety rodent baits were placed in the dwelling, and when the <br /> rodent baits were checked on the following day, it was noticed that each one was opened. <br /> When Mr. Harrington went to the property to meet a contractor to bid on the clean-out of the interior of the <br /> property, the contractor was in the driveway waiting with another man. The other man was from 24 Trauma, <br /> 2 <br />
