Warwick & Assoc. , Inc . , Agent for Eli Harary, Said <br /> revised site plan and explanatory letter are on file with the Mashpee <br /> Conservation Commission." <br /> After review and brief discussion, the following special conditions <br /> were rade a part of the order of Conditions for Project s - , <br /> ohn Halachia being the applicant, to construct a single-family <br /> dwelling and septic system at Lot # u ak r Min Road . <br /> 1. It is duly noted that there are no provisions for access to the <br /> water on plans submitted and as stated at the public hearing ' of <br /> October 25, 1984. <br /> 2 . Hay bales shall be placed and staked along elevation 18 which <br /> is the proposed work limit. <br /> 3. Proposed septic system shall be constructed in accordance with <br /> Title v and the Mashpee Board of Health Ryles and Regulations . <br /> . No mechanized equipment shall be allowed to be used for dri-11ing <br /> the well. <br /> 5 . No clear cutting of the lot beyond the 181 contour will be allowed, <br /> 6 . The Office of the Building Inspector shall be notified at least <br /> twenty-four hours prior to commencement of the work. <br /> . <br /> 7. A copy of the Order of Conditions written and issued for this <br /> project shall be made available at the project site , <br /> 8 . All work shall be performed in accordance with site plans sub- <br /> mitted and approved at the time of the public hearing. <br /> Mr. George Leighton, whose application was heard at public hearing on <br /> September 6, 1984 has requested information regarding the order of <br /> Conditions for this project. <br /> Reference was made to the minutes of that meeting and specifically t <br /> that hearing wherein it is noted that the applicants had not, at that <br /> time filed a Notice of Intent with DEQE. They were given instructions <br /> to do so and subsequently request further hearing when the required <br />
