i <br /> MINUTES March 8, 1990 2 BOARD of HEALTH <br /> 4 <br /> APPOINTMENT: Kenny Anshewitz , Shingle Master <br /> Mr. Anshewitz was in attendance for this scheduled ap- <br /> pointment, <br /> Mr, Anshewitz presented to the Board written authoriza- <br /> tion to operate his business within the towns of Barnstable, <br /> Dennis and Yarmouth. Mr. Anshewitz stated he is present this <br /> evening to request permission to operate his business within <br /> the town of 1 ashpee. one gallon Fantastik per 55 gallons of <br /> bleach is used for shingle cleaning. Approximately 20 to 30 <br /> gallons of bleach are required per house, An electric well <br /> pump is used for application. <br /> Mr. McQuaid stated he consulted with Mr. Dan Sullivan <br /> from . .. . who approves of the operation, as long as, seal- <br /> ers or preservatives are not used in excessive amounts. it. <br /> McQuaid recommended licensing the individual contingent upon <br /> notification to this Board by Mr.. nshewit or an Associate <br /> 48 hrs prior to spraying. This would offer the opportunity <br /> o witness the spraying. <br /> Upon motion duly rade by Mr. Greelish and seconded by <br /> Mr. Doherty voted unanimously to grant a license to fir. <br /> â–ºnshewit d/b/a "Shingle Faster" with the understanding a <br /> license from the Massachusetts Pesticide hoard is not re- <br /> it b quired at this time, the Board of Health reserves the right <br /> to revoke, suspend or modify this approval of your spraying <br /> activity at any time and granting of this license shall be <br /> conditional upon the following requirements. <br /> 1. The only material to be used is a Bleach/Fantastik <br /> mixture. Any alternative method of cleaning will <br /> be reviewed by this Board for approval. <br /> The chemical mixture cannot be sprayed within 1 <br /> feet of any watercourse. The term watercourse in- <br /> cludes any natural or man-made stream, pond, lake, <br /> wetland, coastal wetland, swamp or other body of <br /> water and shall include wet meadows, marshes, <br /> swamps, and bogs and areas where groundwater, <br /> flowing or standing surface water or ice provides a <br /> significant part of the supporting substrate for a <br /> plant community for at least five months of the <br /> year. <br /> Rinsing of equipment or using water from any <br /> watercourse in relationship to the operation is <br /> prohibited. <br /> r <br /> I <br /> q <br />
