Costa, Chairman <br /> 1 . ) Warrants signed <br /> . ) Mr, Doherty motioned to accept the minutes of August 175 1989, Mr. Costa <br /> seconded. <br /> Mr. Doherty motioned to accept the emergency meeting i'nut s. of .August , <br /> 1-989,'L Mr.., Costa seconded. <br /> 3. ) SEMASS commencement notice for signature - Mr. McQuaid commented he <br /> spore with personnel in the Selectmen' s office questioning whe-ther,- in <br /> -the absence of the Executive Secretary whether -the Board of health <br /> should sign the notice. Mr. McQuaid stated his. Mason replied that the <br /> Seiectmen would handle this matter, <br /> 4. ) F 1990 contract with the Visiting Nurses Association - lir. Doherty <br /> and Mr. Gosh forwarded their signatures to the 1990 contract'. <br /> r <br /> APPOINTMENT: Clark Cohen - "The Donut Box" Food permit <br /> Mr. Mark Cohen, Mr. Donald Cohen, Mr. David Cohen and Attorney Elizabeth <br /> Lane, representing the Gohen ' s , were in attendance for this scheduled appoint- <br /> ment. <br /> ppoint- <br /> m nt. <br /> Mr. McQuaid informed the Board Mr.. Peter Lawrence applied for a permit to <br /> open a retail donut outlet at what now is called "1'he Donut. Box. Mr, McQuaid <br /> stated he inspected the premises and determined that due to lack of sanitary <br /> facilities the Cohen' s would be unable to operate a restaurant however a retail <br /> Permit would be permitted for donuts. Mr. McQuaid stated at the time Mr. <br /> Lawrence owned a retail outlet in Falmouth and was informed, at that time,, that <br /> he would' be bringing over bared donuts from the other outlet. Mr. McQuad ex- <br /> plained -the issue of the well was discussed then at which time it was deter- <br /> mined that bottled water would be required to make a of foe and commented a <br /> special permits was issued to Mr. Lawrence from the Board of AppedIS }, hieh <br /> stipulated the bottled water. <br /> Mr. McQuaid relayed to the Board that Mr, Cohen came to the Board of <br /> Health wishing 'to pursue 'the ratter as Mr. Lawrence no i o ger' was interested <br /> in operating the business. Mr. McQuaid explained that Mr. Cohen was informed <br /> at the time that operations of a similar business under the same conditions, <br /> that would have been issued b . , r.
