Mr. Harrington stated that the intern had gone around and completed his <br /> routine inspections. Again he had found that the floor in the kitchen hadn't <br /> been repaired after several years of being noted as a violation, So they <br /> brought the facility in front of the beard. Also there wasn't a servSafe <br /> person at the facility when if was operating. The board requested a show <br /> cause hearing to determine why these items had net been addressed. <br /> Ultimately the board's decision at the meeting two weeks ago was to <br /> have the ServSafe certificate provided for the person who was there full <br /> time. They did receive from the school... <br /> Ms. Warden interjected that she could shover Mr, Santos the letter that <br /> they sent for a quick up to date of the code violations. <br /> Mr. Harrington produced the letter that was seat as a follow up to last <br /> week's meeting. He thea asked Mr. Kogut who "Andrew Cartwright" was. <br /> . <br /> Mr. Kogut answered that the certificate should be for"Andrew Chadwick". <br /> Mr. Harrington then asked Mr. Kogut who "AndrewChadwick" was, <br /> (i can't hear a response from Mr. Kogut. <br /> Mr. Harrington continued by stating that the board basically decided to <br /> request a copy of the sery.Safe certificate from the certified food manager <br /> or the person in chane. They had actually requested that to be issued by <br /> the next day. That did not occur. But, they did receive a fax from the <br /> school, even though the name was not right on the fax... <br /> Mr. Kogut interjected that the school had it right on this one. They <br /> ultimately did fax the right one and apologized for the error and the delay. <br /> Mr. Harrington noted that they did have a copy of Andrew Chadwicks' <br /> ervsafe certificate now, They also had documentation for a licensed <br /> contractor. <br /> Mr. Santos stated that the paper from the contractor was only a proposal. <br /> Mr. Kogut replied that he had signed it and had given it back to <br /> contractor. <br /> Mr. Santos continued by asking when the date was for completion since <br /> the proposal was only good for thirty days. <br /> Mr, Kogut replied that the work was going to be completed by August Wh, <br /> of <br /> g . <br />
