Mr. White seat in reports on 31 Thornberry and 19 Trump Road. There were a <br /> number of deft lencies on the inspection reports. An order letter was sent to Mr. White asking for <br /> a revised report, explaining why the risers and other in pe ti ns were not done. No responses <br /> were sent hack. Nothing has been fixed per the Board's orders to date. . <br /> IIT-. Ball motioned to rescind Mr.Wh'ite's license as of 12/28104 and revoked his license on <br /> 2/17/05. lr. Santos seconded the motion, All agreed. <br /> Mr. White was told to come back before the Board prior to reinstatement of his Septic Inspector's <br /> license. <br /> Mr. White stated that he Inas not yet gone back to 31 Thornberry and 19 Trump Toad. He was <br /> asked why he had not yet corrected the violations. <br /> Glenn Santos made a motion that Mr.White pull a permit and return to 31 Thornberry Road and <br /> 19 Trump Toad within the next two weeks to install risers at no cost to the homeowners. He <br /> shall then contact an Agent of the Board of Health to inspect the installation of the risers and <br /> forward a corrected Septic Inspection Report to the BOH. Steven Ball seconded the motion, and <br /> all agreed. <br /> OLD BUSINESS <br /> CS-10 Flume, MMR Notification <br /> No work has been done to date at the Flume restaurant to install a denitrification system. <br /> Steven Ball made a action that the BOH, in a letter to the Flume Restaurant, revoke their food <br /> permit, and further Mate that no licenses will be issued at that property until the denitrification <br /> system is installed. Glean Santos seconded the motion, and all agreed. <br /> DISCUSSION <br /> 324 Great Neck Road North—Scott Blakeman. <br /> Scott Blakeman reported that Mahoney&Dodass will do the testing for contamination on the site. <br /> He met with Milne Lahute and the Water District. Lines will be split off tQ provide water to the <br /> cottages. Mr. Blakeman an asked for a 30 day extension and was told that he needed to put the <br /> request in writing to the 1301-1.
