On 2/14/06, Veronica <br /> Warden inspected the Gold's Gyres tanning facility and found g violations. Also,-the 2005 tanning <br /> permit had not been paid, but the 2006 tanning permit was applied for, and was paid. on 2116/06, <br /> Veronica Warden reinspected the tanning facility and found an additional 9 violations. on 2/17/06 <br /> certified letter from the BOH was sent to Gold's Gym stating that a Show Cause hearing before the <br /> B H was scl'eduled for3/02/06, at which time Gold's Gym was ordered to provide a Standard <br /> Operating Procedures notebook containing training protocol for all new employees, train in <br /> procedures and sanitizing methods for all tanning rooms and units, as well as eyewear requirements, <br /> an Injury Log Book, and warning notice handouts. The tanning facility was ordered to remain closed <br /> until James loses or a representative from Gold's Gym was in attendance at the next BOH meeting, <br /> On 2121106 Veronica Warden re-inspected the tannin facility acility and found that all violations had been <br /> corrected. As of 2 21106 the Gold's tanning facility was in full 'compliance. <br /> James Doses introduced himself to the Burd. Mr. Dosesal ized for not attending p 9 ng the 2/16/06 <br /> BOH meeting. He further stated that he had 'misunderstood the procedure'to follow to re-applyfor an <br /> annual license, thinking that a letter would.be seat out by the BOH when renewal was due. Mr. Moses <br /> distributed copies of FRONT DESS TANNING PROCEDURES that are now posted at the Gold's G <br /> front desk. Warning signs-are posted in the tannin booths thaty <br /> r Bard: DIE —ULTRA 11oL�T <br /> RADIATION instructions. A TANNING ING iH LOG and a notice for the tanning booths indicating <br /> THIS TANNING UNIT HAS BEEN LEANtD AND SA ITIz <br /> D AND I FAD Fold USE have also <br /> been prepared. Glenn Santos <br /> seconded the motion. All approved. <br /> Pizza Prima—Phil Pana of"`Shover Cause" Hearin <br /> Glen Harrington reported that Phil Panasci was currently on vacation and would riot be attending the <br /> meeting. <br />
