Lewis Mantel <br /> seconded the motion.. All approved. <br /> Glenn Santos made a motion to deny the request by the kotary Club to r i ethe pew it <br /> fee. Levis Mantel seconded the motion. All approved. <br /> Request for Extension to 9OH'Decision: Aapermoon. <br /> Glen Harrington stated that the Board of Health had received a letter dated 9/19/07 from <br /> _ Masa Oyamada of the Papermoon Restaurant, requesting an extension of the deafine <br /> .for Sery Safe certification for the remaining members of their kitchen staff. <br /> Lewis l ant f noted that the.le€t r does not indicate how many ofltheif kilchen.staff, or <br /> whether any of their staff, had received their Sery Safe certification to date. Also, the <br /> letter does not indicate the length of the extension they are requesting. <br /> Glenn Santos made a motion that the BOM;'grant Papermoon a Sear Safe certif16ation <br /> extension until the date of the next 130H meeting, on October 10, 2007. At that meeting, <br /> the ap rrnoon Restaurant rust provide the names of the kitchen staff who have <br /> completed their Sear Safe training., as well as a schedule of the completion of the <br /> training by the remaining kitchen staff. The Health Agent will conduct an inspection prior <br /> to the next BOH meeting to determine whether the restaurant is in cdmplete c mpliance <br /> with BOH regulations.
