All aproved. <br /> :1 M —Stephen Lempitski— septic violation - 102 Captain's Rover <br /> Glen Harrington commented that a letter from the 130H dated 1129108 had been sent to <br /> Stephen Lem itski stating that it had come to the attention of the BOH that the Miorofast <br /> o.5 system-that was installed in 2000 had not been maintained since 2002, and that <br /> therefore Stephen Lernpitski was in violation of the BOH regulation Part IX, section 15, <br /> Innovativel lternative On-site.Septic Systems Reg uiation" which requires ars <br /> operations and maintenance M) contract,, with the required testing and analysis, for <br /> all denitrification units installed in the Town of l ashpee and Title V. Glen Harrington <br /> stated further that Mr. Lernpitski had been notified by the DEP in 2002 of his violation. <br /> The 0 & M contract was received by the 60H, but there was no evidence that there had <br /> been any maintenance or any annual inspections, since 200 2. <br /> Stephen Lempitski, 1 Captain's Fort introduced himself to the Board.
