TOWN OF MASHPEE <br /> BOARD of HEALTH --- PUBLIC MEETING <br /> Minutes of the Meeting-of Wednesday, November 12, 2008 <br /> Board Members Present; Also Present: <br /> Lucy Burton, Chairman Glen Harrington, Health Agent <br /> Burton Kaplan, Vice Chairman <br /> #calliope Eglof#, Clerk <br /> Chairman Lucy Burton called the meeting to order at 7:05 PM. <br /> APPOINTMENTS <br /> 7:15 cape & Islands Engineering—45 Elizabeth Island Ind.- septic Variance <br /> Requests <br /> John Blavinsky, from Cape & Islands Engineering, introduced himself to the Board. Mr. <br /> SIa vinsky requested BOH approval of the following: <br /> 310 CNIR 15.405 Local Upgrade Approval/Maximum Feasible Compliance. <br /> Bottom of soil absorption system to high groundwater will be ' required). <br /> - 310 CII 15.211 Distance Varianoes: the soil absorption system wilI be 2 to the <br /> property line (10' required). The property line will be staked prior to soil <br /> absorption system installation. <br /> Approval of the sin Mair denite system. <br /> Glen Harrington recommended approval of the septic upgrade pleaspresented by John <br /> Slavirisky for 45 Elizabeth Island Road pending receipt of the f llowing: <br /> (1) An Operations & Maintenance contract to include quarterly nitrogen sampling <br /> for 2 years <br /> (2)-A remedial approval letter in compliance with the DEP, including a Barnstable <br /> Counter Registry of Deeds registration to show the presence of an alternative <br /> system, to be provided to the BH prior to issuance of the Certificate of <br /> Compliance. <br /> ti <br /> Burton Kaplan made a motion to approve the plans submitted by John SIa insky for a <br /> septic upgrade at 45 Elizabeth Islam Foad, including the 3 variance requests listed <br /> above, pending receipt of items 1 and 2) as outlined by Glen Harrington above, the <br /> work to be completed no later than 12115/08. Kalliope Egloff seconded the motion. All <br /> approved. <br /> 7:30 Marlene suslenkar—74 Waterway erwary—septic Inspection Variance <br /> Marlene Susienka introduced herself to the Board as representing the owner of 74 Water <br /> Way. ifs. Susienka stated that the owner of the property, Louis Cistoldi, is asking for a <br /> b day extension to bring the septic system into compliance by redirecting the driveway. <br /> The buyer of the property, Al Clemmen , has agreed to do this as soon as he owns the <br /> property. I Ir. Clem mens' closing date on the property is 11 4108. <br /> - 1 <br />
