Burton, Chair
And Burton Kaplan, Co-Chair
16 Great Neck Road
Mashpee, MA 02649
BY FACSIMILE 508-477-0222
11T CLASS US MAIL
RE: 8 Chart Way, Mashpee
Owner Agreement to Upgrade Septic System
Dear Ms. Burton and Mr. Kaplan:
Please accept this correspondence on behalf of Mr. Shane Caiazzo ("Applicant" and
present owner of the above-referenced property), and pursuant to Mashpee BOH regulation Part
IX Section 19 with respect to upgrading the sub-surface disposal system at this site. This
agreement is being submitted in conjunction with the Board's regularly scheduled on this matter
on January 14, 2009. Septic upgrade plans have previously been approved by action of the
Board (BOH approved septic plan 5/17/06 with variances) but not yet completed due to a sale of
the property. The Applicant requests a continuance under the regulations to allow new septic
plans to be approved by this Board, and for the times needed to obtain additional approvals from
the Mashpee Conservation Commission and Board of Appeals allowing for the razing and
replacement of the current dwelling. New proposed septic system plans have been prepared and
submitted to the Board on January 5, 2009 by Engineering Works, Inc. that requires less variance
relief. The property continues to remain unoccupied and the proposed written agreement of the
Applicant is to complete permitting and commence and complete instruction within 18 months
