The BOH agreed to defer any action taken on renewal of the Cavossa Disposal, Inc. <br /> Waste Hauler's Permit for 2009 until after the above-referenced meeting scheduled for <br /> February 13, 2009. <br /> NEW BUSINESS <br /> 1. sign Warrants. Board members signed the warrants. <br /> . Approve Public session Minutes: January 14, 2009 <br /> A motion was made, seconded and passed to approve, as amended, the Public Session <br /> minutes of January 14, 2009. <br /> . Request for 2009 Food Establishment Permit: Wicked ed I estaurant Inc. <br /> Glen Harrington stated that he had reviewed the 2009 Food Establishment Permit <br /> application and the floor plan, submitted by Robert Catania, owner of the proposed new <br /> restaurant located at 3G South Street in South Cape iVillage. The BH has not yet <br /> received.a copy of the Wicked ed restaurant menu. Glen Harrington stated that Mr. <br /> Catania hopes to open the restaurant in March of 2009. <br /> Lucy Burton made a motion to approve the 2009 Food Establishment Permit Application <br /> for the Wicked Restaurant, inc., submitted by Robert Catania, pending receipt of a copy <br /> of the menu, and Board of Health inspection prior to opening of the restaurant. Burton <br /> i <br /> Kaplan seconded the motion. All approved. <br /> . Request for Review and signature of Mutual Aid Agmeement <br /> Glen.Harrington distributed to the Board members a copy of the Mutual Aid Agreement <br /> among Public Health Agencies in Emergency Preparedness Region 5b. Glen Harrington <br /> stated that the agreement is entered into by and between the public health agencies of <br /> the following cities and towns: Barnstable, Bourne,, Brewster, Chatham, Dennis, <br /> Eastham, Falmouth, Harwich, Ilashpee, Orleans, Provincetown, Sandwich, Truro, <br /> 1l ellfleet, Yarmouth, Aquinnah, Chilmark, Edgartown, Gosnold, Oak Bluffs, Tisbury, <br /> West Ti bury, and Nantucket. <br /> Discussion and review of the document by the Board members followed. <br /> Lucy Burton made a motion to approve Mashpee's entering into the Mutual Aid <br /> Agreement Kalliope Egloff seconded the motion. All approved. The Board members <br /> signed and dated the Mutual Aid Agreement. <br /> S. Request for Review of 2009 Refuse hauler Renewal Permits <br /> Glen Harington reported that the Board of Health had received seven Refuse Hauler <br /> Renewal Permit applications for 2009 from the following companies: Cassella Hate <br /> Services, Waste Management, Allied Waste Services, Pina Sanitation Services, cape <br /> Waste, One Pointe Horne curators, Inc., and Macomber's Sanitary refuse Compan .� <br /> Casella Ise Services. Glen Harrington stated that cassella Waste Services <br /> had supplied all the requested information, and had fulfilled all application <br /> requirements for a 2009 permit renewal. <br /> 2 <br /> i -- <br />
