PROJECT SCOPE, PROCUREMENT PLAN and MANAGEMENT <br /> A. Description of Project: Prepare foundations, construct onsite wastewater disposal <br /> facilities, install water service. connect to electrical and communications systems <br /> for placement of two temporary modular buildings on the Mashpee Wampanoar's <br /> Government Center/Powwow site to provide health clinic services and support; <br /> until such time a larger permanent structure will replace these temporary modular <br /> buildings or the Tribe can contract/compact for operation of the facility. <br /> The project consists of the following components: <br /> Work.Package Primary Elements Procurement <br /> Plan <br /> A. Secure land, • Survey land parcel/Site planning PL-94-437 Lease <br /> site work and • Prepare Lease request agreement with <br /> preparation for • IAS NEPA Determination Mashpee Tribe <br /> modular buildings • Construction of septic system &water facilities. <br /> Construction of foundation for modular buildings. <br /> • Place modular buildings on foundation. <br /> • Set-up and connection of all utilities (electric/ <br /> telephone/ water) <br /> • Modifications including handicapped ramps & ADA <br /> compliance in restrooms. <br /> B. Administrative • Design of water service and onsite septic system sized Federal <br /> Building for 2000 SF administration modular building and Procurement <br /> 2700 SF clinic building (will meet Title 5 <br /> requirements). <br /> Transport existing administration building Atmore, <br /> AL to Mashpee, MA <br /> • Procure/deliver office furniture, equipment.&IT <br /> C. Modular Clinic 1 • Design/build/deliver temporary modular building Federal <br /> Building designed for clinic and dental use. Procurement <br /> • Procure/deliver/install medical equipment <br /> • Procure/deliver office furniture &equipment <br /> B. Project Justification: The Mashpee Wampanoag Tribe, a new, federally <br /> recognized tribe, has requested the Indian Health Service provide medical <br /> services to their 2,100 tribal members residing mostly in the service unit area. <br /> This project has been identified as a high priority by the Nashville Area Director <br /> and Chief Medical Officer. <br /> C. Proiect Management: Bangor Field Office OEHE staff will provide quality control <br /> management of preliminary facility design and construction. The Nashville Area <br /> DFE Facilities Engineer will manage and oversee the overall project. <br /> 3 <br />
