Request t for 2014 Food Service and Retail Permits and 2014 Tobacco Permit: Popp] General.Store. <br /> Richard Martini, currently leasing the Poppy General Store, introduced himself to the Board. <br /> Glen Harrington commented that the Food Establishment Permit Application and Tobacco Application have <br /> been submitted for the Poppy General Store. Copies of Mr. Martini's Certified Food Manager Certificate, the <br /> Allergen Awareness Certificate, and the menu have not been provided to the Board of Health. <br /> 1r. Martini remarked that he has the Allergen Awareness Certificate and the menu, and he tested for the <br /> Certified Food Manager's course last week. Mr. Martini will submit a copy of his ServSafe Certificate once he <br /> receives it. <br /> Mr. Harrington stated that the floor plan has not changed from the last occupant, and a final inspection i <br /> required prig to opening. The fee is to be paid upon approval and prig to the issuance of Mr. Martini3s permits. <br /> Burton Kaplan made a motion that the Board of Health approve the 2014 Food Service, Retail, and Tobacco <br /> Permits for Poppy General Store located at 259 Shore give subject to the payment of the.$350.00 permit fees, <br /> the ServSafe Certificate to be provided, and a final inspection by the Burd of Health argent. Kalli pe Egl ff <br /> seconded the motion. The motion was unanimously approved. <br /> OLD BUSINESS <br /> 1. Nitrogen Aggregation Plan: 20 Running Light Way. <br /> Y <br /> Jeff I yther of BSS Design, representing 20 Funning Light Way, introduced himself to the Board. <br /> Mr. l yther commented that all of the edits have been made to the Nitrogen Aggregation Plan that were <br /> requested by Glen Harrington. Mr. I ythe(stated that nitrogen credits will be taken from the property at 27 Aunt <br /> Janes Fuad and added to 20 Funning Light Way so than a 3-bedroom home can be constructed. <br /> Mr. Harrington remarked that the Nitrogen Aggregation Plan for 20 Dunning Light Way was continued from the <br /> last meeting because revised documentation was received at that meeting. Mr. Harrington reviewed the <br /> information and forwarded his comments to BSS Design. The revised plans were provided, and Mr. Harrington <br /> felt that the designs were complete. <br /> Burton Kaplan made a motion that the Board of Health approve the Nitrogen Aggregation Plan for 20 Funning <br /> Light Way.
