This document has important legal consequences; consultation with an attorney is <br /> encouraged with respect to its completion or modification. <br /> SUPPLEMENTAL AGREEMENT <br /> FOR <br /> STUDY AND REPORT <br /> PROFESSIONAL SERVICES <br /> This is an Agreement made as of , 2011, between the Town of Mashpee, <br /> acting through its Board of Sewer Commissioners, 16 Great Neck Road North, Mashpee,MA 02649 (the <br /> COMMISSION) and GIID, Inc. (formerly Stearns & Wheler, LLC), 1545 Iyannough Road, Route <br /> 132,Hyannis, NIA 02601 (ENGINEER). <br /> The COMMISSION employs ENGINEER to perform professional engineering services, to serve as <br /> the COMIvIISSION's professional engineering representative and to provide professional engineering <br /> consultation and advice for a professional fee(as set forth below)in connection with the development of a <br /> Watershed Nitrogen Management Plan / Effluent Pipeline Preliminary Design (the " Project"). These <br /> services are in addition to, those specified in the Agreement made by the same parties on February 2, <br /> 2000. <br /> Except as hereinafter provided, all of the terms and conditions of said February 2, 2000 Agreement <br /> shall remain in effect and are applicable to this Supplemental Agreement. <br /> SCOPE OF SERVICES <br /> The COMMISSION has requested that the ENGINEER provide certain data to the United States Geologic <br /> Survey (USGS), and coordinate with the USGS in preparation of said data, for development of Solute <br /> Transport Modeling of wastewater discharges in conjunction with certain wastewater treatment and <br /> discharge scenarios developed by the ENGINEER for the above-referenced Project. <br /> ENGINEER will provide Basic Services as outlined below: <br /> 1. Attend any necessary meetings or otherwise communicate with USGS to clarify data needs and <br /> format and coordinate preparation of the required data and USGS' modeling results. <br /> 2. Provide the necessary wastewater flow and GIS data relevant to the following discharge scenarios, <br /> with data provided for all parcels in the study watersheds: <br /> 1) existing conditions <br /> 2) GHD's proposed Scenario 1 as presented at the July 19, 2011 meeting of the <br /> COMMISSION,but with existing flows <br /> 3) Said Scenario 1 with future("build-out")flows <br />
