Box 2335 <br /> Mashpee, MA 02649 <br /> RE: Dino's Pizza/Dino's Sports Bar, 414 Nathan Ellis Highway, Mashpee, MA <br /> Dear Mr. Mitrokostas: <br /> On May 11, 2018, Veronica Warden, Assistant Health Agent, conducted a food <br /> establishment inspection, at the above-,stated food establishment, according to 105 CMR 590.000 <br /> of the State Sanitary Code Chapter X "Minimum Sanitation Standards for Food Establishments". <br /> During the inspection, Ms. Warden noted seven non-critical violations and nine critical <br /> violations. <br /> As a result of the inspection with nine critical violations, the facility was placed on three <br /> months of"probation" according to Mashpee Board of Health Regulation, Part IV, Section 5.00. <br /> Per the regulation, a re-inspection shall occur monthly for three consecutive months. <br /> On July 18, 2018, Ms. Warden reinspected the facility for compliance with previous <br /> violations. Ms. Ann Fornal, an employee of the establishment,was present for the re-inspection. <br /> During the re-inspection, Ms. Warden noted three critical violations .and six non-critical <br /> violations. Mashpee Board of Health Regulation, Part.IV, Section 5.00 (III)(2)(C) states, <br /> "Should a monthly inspection report of a food establishment on the Food Establishment <br /> Probation list disclose any critical violations, the food establishment owner shall be issued a <br /> $100.00 fine per critical'violation." On July 18, 2018, the food establishment was documented <br /> with three critical violations during the probationary period. Therefore, a fine of $300.00 is <br /> issued for-non-compliance. <br /> During the,previous four food inspections on March 28, 2018, May 11, 2018, June 1, <br /> 2018 and July 18, 2018, two non-critical violations were cited and failed to be repaired. In . <br /> accordance with Section 5.00 (11I)(1)(D), $25.00 fines shall be issued for non-critical violations <br /> not repaired within the specified timeframe. Therefore, a $150.00. fine is issued for failing to <br /> correct two non-critical violations at three subsequent food establishment inspections. <br />
