%7V <br /> Ice� . <br /> . <br /> .. �I Town o Mashpee <br /> 16 Great Veck &ad Xorth <br /> Mashpee, Massachusetts 02649 <br /> Grievance Procedures <br /> for <br /> Handicapped Persons <br /> It is thep olicy of the Town of Mashpee that it shall not discriminate against any person on the <br /> basis of race, creed, color, religion, sex, national origin, age or physical or mental handicap in the <br /> provision of Town facilities, programs, employment or services. <br /> Therefore, the Board of Selectmen of the Town of Mashpee, in order to ensure that said policy is <br /> being met, have established the following procedures for the filing of grievances against any <br /> agency of the Town which is believed to have violated said policy with regard to physical or mental handicap. These procedures are in addition to the grievance procedures for Job applicants <br /> and employees outlined in the Town's Affirmative Action Policy, adopted Dec. 18, 1995. <br /> A. Who may file Anyone who believes that she or he has been subjected to discrimination on <br /> the basis of a handicap may file a complaint. A representative may also file a complaint on <br /> behalf of persons who feel that they have been discriminated against because of their <br /> handicap. <br /> B. How to file All complaints must be in writing on a form entitled Town of.Mashpee <br /> HandicapDiscrimination Complaint, which may be obtained from the offices of the Board <br /> p <br /> of Selectmen, the Town Clerk or the Personnel Board, and which may be accompanied by <br /> additional written or photographic evidence or explanation. The complaint form must be <br /> signed by the complainant and/or the complainant's representative. In the event that a <br /> complaint is made in other than written form, the official receiving the complaint must <br /> instruct the complainant to reduce the complaint to writing on the proper form. <br /> C. where to file Upon completion of the required form, the complaint should be filed either <br /> with the Executive Secretary at the office of the Board of Selectmen, or by mail to the <br /> Executive Secretary at the following address: <br /> Town of Mashpee <br /> Executive Secretary <br /> Great Neck Road North <br /> Mashpee, MA 02649 <br /> �k <br /> 1 ? <br /> All complaints must be filed within 180 days of the date of the alleged <br /> D.
