MASHPEE ZONING BOARD OF APPEALS <br /> DECISION FOR A SPECIAL PERMIT <br /> Thomas F. Smith and Bonnie Ann Smith <br /> 266 Monomoscoy Road (Map 120 Parcel 57B) <br /> Mashpee, MA 02649 <br /> SP-2015-21 <br /> Mr.John Slavinsky,represented the applicants for the proposed project.At the prior <br /> meeting, the Board stated there were three pending issues that needed to be cleared up <br /> before a decision could be made. These issues were as follows; <br /> -Board of Health$800 fine. <br /> -Service contract for septic system. <br /> -Harbormaster requirements and conditions. <br /> Jonathan confirmed with the applicant that the float will be 8' x 25' feet, and there <br /> were no issues with the Harbormaster. John Slavinsky said the Harbormaster requested the <br /> specific location for the dock as depicted on the plan. The Conservation Order of <br /> Conditions was recorded at the Barnstable Registry of Deeds as Book#28002 Page 206. <br /> Jonathan read the Board of Health comments into the record; "We received the <br /> contract for the I/A system at 266 Monomoscoy Road on Friday, 17 July 2015.", and an <br /> email dated July 2, 2015: "The$800.00 fine has been paid in full and deposited. I checked <br /> with the operator of the composting toilet at 266 Monomoscoy Rd and he said that he will <br /> supply a contract for 2015 when he gets back from vacation. The Smiths have the rest of <br /> the year to comply with the annual maintenance of their system." <br /> Jonathan read the comments from the Harbormaster into the record; "The <br /> Harbormaster requires that the dock/floats including l0A floats must be updated to black <br /> hard plastic encapsulated block flotation and all the foam/Styrofoam be removed and <br /> disposed of properly. This will help eliminate the issue of storm bourn debris on our bays, <br /> lakes and rivers. Also that the street address must be permanently and clearly displayed <br /> with minimum of 3"block letters with contrasting color at end of pier/piling for emergency <br /> purposes, and in addition to the owners address be pennanently affixed on all floats and <br /> ramps with 3" block letters with contrasting color. Otherwise, The Harbormaster has no <br /> issue with this plan." <br /> GENERAL CRITERIA: <br /> The subject property located at 266 Monomoscoy Road is approximately 18,500 <br /> square feet. <br /> There were no abutters present. <br /> 2 <br />
