Town of fda;bpee <br /> t�yCy-( y BOARD OF HEALTH <br /> 16 GREAT NECK ROAD NORTH <br /> MASHPEE, MASSACHUSETTS 02649 <br /> (508) 539-1426 <br /> November 10,2014 <br /> Mr.Raul Lizard!-Rivera,P.E. <br /> Holmes&McGiath,Inc. <br /> 205 Worcester Court <br /> Falmouth,MA 02540 <br /> Re: 89 Industrial Drive,Mashpee,MA(Assessor's map 88,parcel 27) <br /> Dear Mr.Lizardi-Rivera: <br /> On November 6,2014,the Mashpee Board of Health reviewed the proposed <br /> commercial site and septic plan("Septic Plan Prepared for the Accidental Auto Body for <br /> Lot 26, #89 Industrial Drive in Mashpee" dated September 24,2014,revised October 24, <br /> 2014, sheets 1 through 3)for the approval of the commercial use and proposed septic <br /> system. Mark Gildea,Esq.,was also present to represent the project to the board. The <br /> proposed plans were prepared for constimeting a 9,000 square foot building for auto body <br /> repair purposes at 89 Industrial Drive. <br /> After a discussion,the Masbpee Board of Health approved the site and septic plan <br /> with the following conditions: <br /> • The MDC catch basins and forebays and bio-retention areas shall be monitored <br /> and maintained to mitigate standing water annually. An executed contract for <br /> servicing of the MDC-type catch basins and bio-retention areas shall be <br /> provided to the Board of Health prior to issuance of Certificate of Occupancy. <br /> • Safety Data Sheets (SDS) shall be kept on premises in bound notebooks for all <br /> hazardous products. The MSDS shall be available for inspection by an agent <br /> of the Board of Health. <br /> • No long term storage of damaged autos or used parts outside. <br /> • Discharge from the spray booth shall be on the not-them end of the building <br /> away from residential neighborhoods. <br /> • A contract for the pumping and disposal of the industrial tight tank contents to <br /> be provided to the Board of Health. <br /> • A contract to maintain the catch basins and retention areas be provided. <br /> • No auto body work shall occur outside of the building. <br /> 5 <br />
