On July 28, 2023, during a re-inspection to verify compliance, Ms. Willander <br />noted that the hand-wash sink was not accessible, blocked by a trash container and other <br />items, and contained dirty dishes. <br /> <br />On August 9, 2023, during a re-inspection to verify compliance, Ms. Willander noted that <br />the hand-wash sink was not accessible, due to a trash can and other items stored directly in <br />front of it, and a bucket stored in the basin. <br /> <br />Siena accepted responsibility for the violation and noted that it was their failure to keep the <br />area clear and accessible. Siena made clear that anyone from the BOH was welcome in their <br />kitchen at any time. To remedy this problem, Siena informed the BOH that it has moved 6 <br />pieces of equipment to make sure that this violation does not occur again. Siena has <br />operated for 20 years without violation and is an integral part of the community. Mr. <br />Seabury was unaware that corrective actions were taken and went through different <br />options the BOH could enact to address the situation. Mr. Virgilio was very troubled by the <br />number of violations – 4. Mr. Raposa concurred that after one violation there would have <br />been immediate correction – moving the equipment. Mr. Livingston noted the hectic nature <br />of a kitchen but was also troubled by the number of violations. Siena reiterated his/her <br />contrition and assured the BOH it would not happen again. <br /> <br />Mr. Raposa made a motion to institute the $100 fine and subject to follow-up re-inspection <br />to ensure compliance, which was seconded by Mr. Livingston. <br /> <br />Mr. Livingston (Yes), Mr. Raposa (Yes), Mr. Virgilio (Yes) – Motion passed unanimously. <br /> <br /> <br />NEW BUSINESS: <br />st <br />1. Review and Approval of Minutes – August 1, 2023 <br /> <br />Mr. Livingston made a motion to accept the minutes as presented, which was seconded <br />by Mr. Rapoasa <br /> <br />Ed Raposa (Yes), John Livingston (Yes), Ernest Virgilio (yes) Motion passed unanimously. <br /> <br />2. Expense Warrants and Adoption of New Warrant Approval Procedure <br /> <br />The warrants were presented and signed/approved by the Members. (Massachusetts <br />Health Officers Association training for two days covering all certificates required, VNA <br />costs, and for the National Night Out table costs) <br /> <br />Regarding the procedure for the expense warrants, a new procedure was adopted <br />where the BOH would designate the Health Agent to approve the warrants and simply <br />update the BOH at the following meeting. <br /> <br /> <br />
