If,however, you have either become incorporated or are now operating <br /> under your complete surname (e.g. John Doe Construction), a Business Certificate is no longer <br /> required. However, if you are no longer in business, please complete the Statement of Discontinuance. <br /> It is important that you complete the enclosed Business Certificate Application. Also, the <br /> Business Certificate checklist must be completed.
