SECTION VI <br /> HEALTH(School Nurse) <br /> The Nurse is on duty during the entire school day. Students who feel ill in the school must have <br /> authorization from staff to visit the Nurse's Office. <br /> Absence -When a child is absent, it is the responsibility of the parent or guardian to telephone <br /> 508-539-1526 by 8:45am to inform the school that the child will not be attending that day. <br /> In addition to phoning in an absence, parents/guardians must send in an absentee note with the child <br /> upon returning to school after an absence. <br /> Emergency Card—The emergency card needs to be updated immediately if you have any changes in <br /> your employment,home phone number,home address,cell phone number or emergency contact <br /> persons (two local individuals). <br /> Be aware that your emergency contact person will be contacted and responsible for your child in the <br /> event of an injury or illness if we are not able to reach you. If the individual who comes to school to <br /> pick up your child is not on the emergency card,but has been designated by you for pickup,he/she <br /> will be required to produce a picture ID upon arrival to school. If no one can be reached in the event <br /> and our child will be transported to the nearest hospital. <br /> of an emergency, 911 will be called, y P <br /> Parents/guardians will be notified, as needed, throughout the school year of occurrences of <br /> communicable diseases. Under certain circumstances, further information will be disseminated <br /> through the Connect Ed computerized notification system. <br /> Dismissal for illness -The Nurse will determine whether the student is not well enough to remain in <br /> school and the parent/guardian will be contacted. Parents/guardians are required to list two alternate <br /> names on the Emergency Procedure Cards of persons to be contacted if the parent/guardian is <br /> unavailable. <br /> Immunization - It is the parent's/guardians responsibility to provide immunization and subsequent <br /> booster information at the time of registering a child in school. When a student does not meet full <br /> immunization/physical exam requirement according to the Massachusetts Department of Public Health <br /> and Massachusetts General Laws, Chapter 76, Section 15, the following steps will be taken: <br /> • Parents/guardians will receive a phone call from school personnel regarding the missing required, <br /> information. <br /> • The student will be excluded from school. <br /> Proof of immunizations for Massachusetts Public School students is required for diphtheria, pertussis, <br /> tetanus, polio, measles, mumps and rubella. (Chapter 76, Section 15) <br /> Lead Poisoning— Parents/guardians must present documented evidence of lead poisoning screening <br /> prior to entrance into kindergarten(105 CHR 460). <br /> 51 <br />
