947739 FORM MASS. 783 8-67 ORDER FOR BURIED CABLE LOCATION By the Board of Selectmen of the Town of MASHPEE Massachusetts ORDERED: That permission be and hereby is granted the NEW ENGLAND TELEPHONE AND TELEGRAPH COMPANY to place and maintain buried cable, manholes, handholes, and pedestals with„the wires and cables therein, markers and other fixtures in or under the surface of'the following public way or ways, as requested in petition of said Company dated the 20th day of JULY 19 83 SAMPSON'S MILL ROAD TWO (2) PEDESTALS (ONE PEDESTAL TO BE REMOVED) PROPOSED BURIED CABLE RUNNING EASTERLY FROM TIE IN EXISTING CABLE BY PEDESTAL TO BE REMOVED AT PROPOSED CAPE DRIVE, 100' TO NEW PEDESTAL ON NORTH SIDE OF ROAD. PROPOSED BURIED WIRE RUNNING EASTERLY FROM EXISTING PEDESTAL ON SOUTH SIDE OF ROAD 120' TO PROPOSED PEDESTAL AT POLE 18/18 ENDING. Substantially as shown on plan marked-N. E.,T. & T. CO. No...........................a3-571........................................ , Dated...................7-20-83......... filed with said petition. Also that permission be and hereby is granted said NEW ENGLAND TELEPHONE AND TELEGRAPH COMPANY to place and maintain underground laterals, manholes, handholes, pedestals, cables and wires in the above or intersecting public ways for the purpose of making connections with such poles and buildings as it may desire for distributing purposes. The foregoing permission is subject to the following conditions: 1. The laterals, manholes, handholes,pedestals,and markers shall be of such material and construction and all work done in such manner as to be satisfactory to such municipal officers as may be appointed to the supervision of the work, and a plan showing the location of buried cable laid shall be filed with the Town when the work is completed. Z. Said Company shall indemnify and save the Town harmless against all damages, costs and expense whatsoever to which the Town may be subjected in consequence of the acts or neglect of said Company, its agents or servants, or in any manner arising from the rights and privileges granted it by the Town. 3. In addition said Company shall before a public way is disturbed for the placing of its buried cable, execute its bond in a penal sum of Five Thousand Dollars ($5,000) (reference being had to the bond already on file with said Town) conditioned for the faithful performance of its duties under this permit. 4. Said Company shall comply with the requirements of existing by-laws and such as may hereafter be adopted governing the construction and maintenance of buried cable, conduits and wires so far as the same are not inconsistent with the laws of the Commonwealth.
