Mas Form 553 <br /> 9-81 ORDER FOR CONDUIT LOCATION J�oui/l/ <br /> In Board of Selectmen of the Town of MASHPEE Massachusetts. 932916 <br /> ORDERED: <br /> That permission he and hereby is granted the NEW ENGLAND TELEPHONE AND TELEGRAPH <br /> COMPANY to lay and maintain underground conduits and manholes, with the wires and cables to he placed <br /> therein, under the surface of the following public way or ways as requested in petition of said Company <br /> dated the 5th day of JULY 19 89 <br /> QUINAQUISSET AVE. PROPOSED: <br /> (WILLOWBEND DR.) <br /> TWO DUCTS BEGINNING AT POLE 22/15, RUNNING EASTERLY <br /> APPROX. 256' TO PROPOSED PEDESTAL. ONE DUCT CONTINU– <br /> ING FROM PEDESTAL, CROSSING WILLOWBEND DR. TO DEV. <br /> APPROX. 2521 . <br /> ONE (1) PEDESTAL <br /> Substantially as shown on plan marked—N. E. T. & T. CO., No. 89-581 Dated 7-5-89 <br /> filed with said petition. <br /> Also that permission be and hereby is granted said NEW ENGLAND TELEPHONE AND TELE- <br /> GRAPH COMPANY to lay and maintain underground conduits, manholes, cables and wires in the above or <br /> intersecting public ways for the purpose of making connections with such poles and buildings as it may desire <br /> for distributing purposes. <br /> The foregoing permission is subject to the following conditions: <br /> 1. The conduits and manholes shall be of such material and construction and all work done in such manner <br /> as to be satisfactory to the Board of Selectmen or to such officers as it may appoint to the supervision of the <br /> work, and a plan showing the location of conduits constructed shall be filed with the Town when the work L <br /> completed. <br /> 2. In every underground main line conduit constructed by said Company hereunder one duct not less than three <br /> inches in diameter shall be reserved and maintained free of charge for the use of the fue and police telephone <br /> and telegraph signal wires belonging to the Town and used by it exclusively for municipal purposes. <br /> 3. Said Company shall indemnify and save the Town harmless against all damages, costs and Apense <br /> whatsoever to which the Town may be subjected in consequence of the acts or neglect of said Company, its <br /> agents or servants, or in any manner arising from the rights and privileges granted it by the Town. <br /> 4. In addition said Company shall, before a public way is disturbed for the laying of its wire or conduits, <br /> execute its bond in a penal sum of Five Thousand Dollars ($5,000) (reference being had to the bond already <br /> on_file with said Town) conditioned for the faithful performance of its duties under this permit. <br /> 5. Said Company shall comply with the requirements of existing by-laws and such as may hereafter be adopted <br /> governing the construction and maintenance of conduits and wires, so far as the same are not inconsistent with <br /> the laws of the Commonwealth. <br /> I hereby certify that the foregoing order was adopted at a meeting of the Board of Selectmen of the Town <br /> MASHPEE25th. . . . .......day of..August.......19 89 <br /> of................__......._......_........_._..._.Massachusetts, held on the................ <br /> .... .. . <br /> _ ✓. _. ................................r............ <br /> Carol H.
