<br />      			5.  Conditioned on all Town department comments read into the record to become part
<br />   				of the Special Permit.
<br />      			6.  Conditioned that the Special Permit be specifically referenced and become part of
<br />   				the condominium documents including the Town department comments.
<br />      			7.   Conditioned  that  the  Special  Permit  will  specifically  be  referenced  in  the
<br />   				condominium documents at the time of submittal for a building pen-nit.
<br />      			8.  Prior to the request of an occupancy permit by the applicant, the applicant will
<br />   				prove to the Zoning Enforcement Officer that the prior mentioned condominium
<br />   				documents have been recorded at the registry of deeds.
<br />      			9.  Contingent upon final approval by the Board of Health and their stipulations that
<br />   				were read into the record, and that their comments are included as an Attachment
<br />   				to the Special Permit Decision.
