Experience/Credentials: How Will the.experience of the applicants(s) <br /> contribute to the success of this project? <br /> A list of board members is attached. <br /> The members of the FORWARD board of directors have a range of backgrounds and expertise. <br /> Most have a close family member with autism, have social work or government experience, and <br /> several also have construction and nonprofit fundraising expertise. To execute the project, we have <br /> assembled a wide-range of highly skilled partners and collaborators. <br /> We are very proud of the partnerships we have already established and grateful for the support of <br /> our local communities, the state and municipal funding we've received to date, and the critical help <br /> we have and will continue to receive from other Cape nonprofit human services organizations, <br /> most notably g the Housing Assistance Corporation, Cape Abilities, and the state's Department of <br /> Developmental Services. <br /> Partners include: <br /> A. Town of Dennis CPC. The Town has committed $500,000. <br /> B. Town of Barnstable CPC.The Town has committed $250,000. <br /> C. Town of Yarmouth CPC. The Town has committed $120,000. <br /> D. Town of Chatham CPC. The Town has committed $50,000. <br /> E. Town of Brewster CPC. The CPC has approved $75,000. Final approval is pending <br /> November Town Meeting. <br /> F. Housing Assistance Corporation HAC HAC has provided fundraising assistance and <br /> consulting and will provide development and construction consulting throughout the <br /> project. <br /> G. Cape Abilities, Cape Abilities,'under contract with FORWARD, will provide around-the- <br /> clock customized services to the residents, including services rvices mandated by the <br /> se <br /> Department of Developmental Services. <br /> H. Town of Dennis.The Town is providing a,99-year lease of 5 acres of town owned land for <br /> the project at nominal cost. It is estimated that this arrangement will save FORWARD well <br /> over$500,000 in land acquisition costs.The Town of Dennis is also providing ongoing <br /> permitting,, construction and legal assistance through its planning, legal and engineering <br /> departments. <br /> 1.
