MINUTES June 16, 1994 5 BOARD OF HEALTH Ms. Peters relayed, "There will be a total of 17 food stands at the Pow-Wow. " "We have leased twelve portable toi- lets- " "Two of the stands have portable generator's." Mr. Evans relayed, "Mr. McQuaid and I inspect the pre- mises daily. " Mr. Doherty requested a consumer sign to be placed on each food stand. Mr. Evans reminded Ms. Peters to have an adequate amount of dumpsters available. Mr. Doherty motioned to grant 17 temporary food stand permits to Mashpee Wampanoag Tribal Council effective July 2 , 3 & 4 , 1994 and a fee of $25.00 per stand will be charged for inspection services, Mr. Cram seconded, all agreed. *****NEW BUSINESS***** Request for food service license . "Willowbend loth Hole Snack Bar - Mr. Evans relayed, "This will be a seasonal operation. " Mr. Doherty motioned to grant a 1994 seasonal food ser- vice license to the "Willowbend 10th Hole Snack Bar", Mr. Cram seconded, all agreed. Cigarette Vending Machines - Mr. Bali directed the Agents to remove any cigarette vending machines located in town. APPOINTMENT: Tom Barboza - 25 Uncle Henry's Road Mr. Tom Barboza, installer and Mr. Michael LeBlanc, builder, were in attendance for this scheduled appointment. Mr. Leblanc stated, "The owner retained his own in- staller to install the septic system. " "We had not idea an- other installer had signed off on the permit. " "My in- staller, Tom Barboza, at the same time was involved in three or four systems and had forgotten to pull the permit for this system. " "The system was inspected by the engineer as re- quired however, he forget to pull the $50.00 permit. " Mr. McQuaid relayed, "The installer is aware of our re- quirements. " Mr. Barboza stated, "It was an honest mistake. " Mr. McQuaid relayed, "I would recommend Mr. Barboza pull the permit, pay the fee and if it happens again revoke his license. "
