MINUTES June 16, 1994 5 BOARD OF HEALTH <br /> Ms. Peters relayed, "There will be a total of 17 food <br /> stands at the Pow-Wow. " "We have leased twelve portable toi- <br /> lets- " "Two of the stands have portable generator's." <br /> Mr. Evans relayed, "Mr. McQuaid and I inspect the pre- <br /> mises daily. " <br /> Mr. Doherty requested a consumer sign to be placed on <br /> each food stand. <br /> Mr. Evans reminded Ms. Peters to have an adequate amount <br /> of dumpsters available. <br /> Mr. Doherty motioned to grant 17 temporary food stand <br /> permits to Mashpee Wampanoag Tribal Council effective July 2 , <br /> 3 & 4 , 1994 and a fee of $25.00 per stand will be charged for <br /> inspection services, Mr. Cram seconded, all agreed. <br /> *****NEW BUSINESS***** <br /> Request for food service license . "Willowbend loth Hole Snack <br /> Bar - Mr. Evans relayed, "This will be a seasonal <br /> operation. " <br /> Mr. Doherty motioned to grant a 1994 seasonal food ser- <br /> vice license to the "Willowbend 10th Hole Snack Bar", Mr. <br /> Cram seconded, all agreed. <br /> Cigarette Vending Machines - Mr. Bali directed the Agents to <br /> remove any cigarette vending machines located in town. <br /> APPOINTMENT: Tom Barboza - 25 Uncle Henry's Road <br /> Mr. Tom Barboza, installer and Mr. Michael LeBlanc, <br /> builder, were in attendance for this scheduled appointment. <br /> Mr. Leblanc stated, "The owner retained his own in- <br /> staller to install the septic system. " "We had not idea an- <br /> other installer had signed off on the permit. " "My in- <br /> staller, Tom Barboza, at the same time was involved in three <br /> or four systems and had forgotten to pull the permit for this <br /> system. " "The system was inspected by the engineer as re- <br /> quired however, he forget to pull the $50.00 permit. " <br /> Mr. McQuaid relayed, "The installer is aware of our re- <br /> quirements. " <br /> Mr. Barboza stated, "It was an honest mistake. " <br /> Mr. McQuaid relayed, "I would recommend Mr. Barboza pull <br /> the permit, pay the fee and if it happens again revoke his <br /> license. " <br />
