February 29, 2008 <br /> Mr. John Slavinsky <br /> Cape & Islands Engineering <br /> 800 Falmouth Road, Suite 301C <br /> Mashpee, MA 02649 <br /> Re: 64 Industrial Drive, Mashpee, MA (Assessor's map 88, parcel 34) <br /> Dear Mr. Slavinsky: <br /> On February 13, the Mashpee Board of Health reviewed the proposed commercial <br /> site and septic plan ("Proposed Septic Plan located in Mashpee, Massachusetts, Prepared <br /> for the Kelliher Group" dated January 3, 2008, sheets 1 and 2). The proposed plans were <br /> prepared for constructing a 20,000 square foot building for office (14,000 square feet) <br /> and industrial purposes at 64 Industrial Drive. The proposed Title V design flow is 1,140 <br /> gallons per day. — — — — — — <br /> The Mashpee Board of Health approves with the following conditions: <br /> The MDC catch basins and forebays and bio-retention areas sha e <br /> monitored and maintained to mitigate standing water annually. An execute <br /> contract for servicing of the MDC-type catch basins and bio-retention areas <br /> _shall— , oard of Health prior to issuance of Certificate of <br /> Occupancy. <br /> • Floor drains are not allowed unless in compliance with state and local <br /> regulations. <br /> • The stone forebay outlets—of— e io-retention areas shall be installed to <br /> mitigate standing water by maintaining the same elevation as the lowest point <br /> within the forebay. <br /> • Hazardous materials shall be limited in volume to 200 pounds dry weight and <br /> 220 gallons of liquid storage. Hazardous waste shall be limited according to <br /> 310 CMR 30.00, Massachusetts Hazardous Waste Regulations. Storage of <br /> hazardous materials/waste outside is prohibited. <br />
