MASHPEE HIGH SCHOOL BID EVALUATION FORM MASHPEE, MASSACHUSETTS EQUIPMENT BIDS - 2/8/96 PAGE 2 of 2 SECTION 105 - CAFETERIA FURNITURE NOTES-BID QUALIFICATIONS: ROBERT H.LORD COMPANY: 1.) Delivery by August 1996, if order is received by March 1996. See cover letter attached to bid for individual manufacturers' delivery lead times. 2.) All items bid in manufacturers' standard colors and construction. 3.) In the case of any construction delay, R. Lord Co. requires 30-days advance notice prior to requested ship date in order to change delivery date. 4.) Should a delay warrant storage charges, the fee will be $500.00 per month with a one-month minimum. 5.) Re-delivery charge from the storage facility to the site will be billed at $25.00/man, per hour. 6.) If items are to be off-loaded at the site and put into storage to be installed/assembled at a later date, the Town will be charged for the extra trip at a rate of $25.00/man hour, including travel time. SCHOOL SPECIALTY: 1.) Delivery is approximately 3-7 weeks ARO. 2.) Pricing is guaranteed for 60 days. 3.) All Krueger items bid in the standard manufacturer's colors. OFFICE RESOURCES: 1.) Delivery, as requested. 8 week lead-time. MASS. CORRECTIONAL INDUSTRIES: 1.) Bid did not include a bid surety. PREFERRED SEAT: 1.) Delivery, as requested. MONITOR EQUIPMENT COMPANY: 1.) Delivery is 60-90 days ARO. EQUIPMENT ENVIRONMENTS: 1.) Bid prices for Item CF-01 & CF-02 includes chrome powdercoat paint finish. ADD $1,700 for plated chrome. 2.) Delivery is 6-8 weeks ARO/colors. If plated chrome option is selected, ADD 2-4 weeks for delivery. SLAYCO, INC.: 1.) Bid prices include delivery and installation. 2.) Bid prices will hold for 90 days. 3.) All items are bid in manufacturer's standard colors, finishes and construction. 4.) Items CF-01 and CF-02 are bid with chrome frame and perfect edge. 5.) Item CF-02 has been bid as an alternate round table in lieu of an oval table.
