MINUTES April 12, 2001 5 BOARD OF HEALTH <br /> Ms. Warden stated that she wants to see plans showing one three-bedroom house on <br /> the combined lots. She further stated that the Department of Environmental <br /> Protection would need to approve the plans for the septic system with a FAST unit for <br /> provisional use. <br /> Mr. Santos motioned to approve the plan for one three-bedroom home on the <br /> combined lots with a FAST system and state approval for provisional use. Mr. Doherty <br /> seconded the motion. Motion passed. <br /> DISCUSSION <br /> a. Smoking Ban Petition <br /> Discussion took place regarding the petition for a smoking ban. Board <br /> members decided to take no stand on the issue. <br /> 8. Revised plans - 227 Great Pines Drive (previously denied by Board for lack of <br /> abutting well delineation) <br /> The applicant submitted a variance request letter without any specific variances <br /> noted. Board members agreed to continue this agenda item until a corrected <br /> cover letter outlining specific variances and a plan with the required locus is <br /> received. <br /> 9. Revised plans - 1 Waterline Drive North (continued pending pump system <br /> design, distance from tank to house and I/A tech. system) <br /> The applicant is requesting the installation of a FAST system for the septic <br /> system at 1 Waterline Drive North. <br /> Mr. Santos motioned to approve the installation of a FAST system for <br /> provisional use pending DEP concurrence for 1 Waterline Drive North. Mr. <br /> Doherty seconded the motion. Motion passed. <br /> 10. Emergency Failure Certifications <br /> 41 Cranberry Avenue <br /> Health Consultant Charlotte Stiefel indicated that the leach pit was full and <br /> overflowing. Ms. Warden stated that the installation of 2 500-gallon leaching <br /> chambers and a new distribution box is necessary. <br /> Board members forwarded their signatures to the Emergency Failure <br /> Certification form. <br /> 7 Nashawena Road <br /> Ms. Stiefel indicated that the leach pit is overflowing and that she is requiring a <br /> perc test and engineered plans based on the proximity to the ocean, the soils <br /> and due to the repair that was done in 1992. <br /> Board members forwarded their signatures to the Emergency Failure <br /> Certification form. <br />
