MINUTES March 29, 2001 4 BOARD OF HEALTH <br /> Mr. Doherty motioned to continue this item until revised plans are received. Mr. <br /> Santos seconded the motion. Motion to continue passed. <br /> 11. Response from Kopelman & Paige re: Smoking Ban Petition for Annual Town <br /> Meeting (as well as other related correspondence) <br /> Board acknowledged. <br /> 12. Definitive Subdivision Plan Approval - Anthony's Way (Harbor Homes) <br /> Mr. Santos motioned to approve subdivision plans for Anthony's Way. Mr. <br /> Doherty seconded the motion. Motion passed. <br /> 13. Variance request - 399 Monomoscov Road <br /> The applicant is requesting a 5' variance from the soil absorption system and <br /> reserve from the property line, a 1' variance from the soil absorption system to <br /> the slab foundation, and to design the soil absorption system with a 19.4% <br /> reduction maintaining the 5' separation to the adjusted groundwater. <br /> Board members agreed to continue this agenda item until Ms. Warden can <br /> verify the number of existing bedrooms in the dwelling. <br /> 14. Emergency Failure Certifications: <br /> 5 Vista Circle <br /> Ms. Warden indicated that the leach pit is full and that the installation of 2 <br /> 500-gallon drywells with 3' of stone and a new distribution box is necessary. <br /> Board members forwarded their signatures to the Emergency Failure <br /> Certification form. <br /> 48 Oregon Road <br /> Ms. Warden indicated that the leach pit is full and the tank is overflowing. Due <br /> to the close proximity to wetlands, a perc test and engineered plans are being <br /> required. <br /> Board members forwarded their signatures to the Emergency Failure <br /> Certification form. <br /> 17 Deer Ridge Road <br /> Ms. Warden indicated that the leach pit is full into the riser and that the <br /> installation of 2 500-gallon drywells with 3' of stone and distribution box is <br /> necessary. <br /> Board members forwarded their signatures to the Emergency Failure <br /> Certification form. <br /> 15. Johnny's Tune & Lube <br /> Plans have been received for a new gas station and auto repair facility at 723 <br /> Falmouth Road. Ms. Warden had not had the opportunity to review the plans, <br /> therefore this agenda item was postponed. <br />
