e 1 <br /> Glen Harrington commented that a variance has been requested to allow an irrigation well to be installed at <br /> 110 Wheeler Road in an area of known or anticipated groundwater contamination. The property lies within the <br /> plume of CS-10 so VOC (volatile organic compound) testing is warranted on an annual basis. <br /> Mallory Langler made a motion that the Board of Health approve the irrigation well at 110 Wheeler Road with <br /> the following conditions: <br /> 1. The annual VOC testing be performed. <br /> 2. An affidavit be recorded in the Registry of Deeds that requires the VOC testing. The affidavit should <br /> include the fact that the owner should acknowledge presence and risks from exposure of the PFC's <br /> and that the owner has read and understood the EPA, DPH and CDC guidance documentation on <br /> PFC's. The affidavit must state that the irrigation well may be discontinued or abandoned under orders <br /> from the Board of Health, if required. <br /> 3. There should be no spigots for the system. It should be a below ground system only. <br /> Laurel Almquist seconded the motion. The motion was unanimously approved. <br /> 7.35 PM —Operation and Maintenance Violation -- 55 Pequot Avenue. <br /> Steve Jarvis, owner of the property at 55 Pequot Avenue, introduced himself to the Board. <br /> Glen Harrington stated that the property located at 55 Pequot Avenue had a septic system that was approved <br /> and installed on April 7, 2010. The Jarvis' purchased the property on May 17, 2010. An Operation and <br /> Maintenance Contract was provided by the seller, which stated that the buyer would be assuming the <br /> responsibilities. To date, no evidence of servicing of the septic has been provided to Barnstable County or the <br /> Board of Health. <br /> Mr. Jarvis commented that he had no knowledge that the septic system required maintenance nor did he <br /> receive notification. Once Mr. Jarvis received notice from the Board of Health, he immediately obtained a <br /> contract with Cape Septic Inspections. Mr. and Mrs. Jarvis did not live at the property full time until two years <br /> ago. <br /> The Board of Health did not take action as there was no evidence of prior notification to perform the sampling <br /> and the operation and maintenance for the septic system. <br /> NEW BUSINESS <br /> 1. Sign Expense Warrants. The'Board members signed the expense warrants. <br /> 2. Review/Approve BOH Public Session Minutes: September 7, 2017. <br /> Mallory Langler made a motion that the Board of Health approve the Public Session minutes of September 7, <br /> 2017. Laurel Almquist seconded the motion. The motion was unanimously approved. <br /> 3. Request for 2017 Bodywork Practitioner License— Ellen Demers Wakefield. <br /> Laurel Almquist commented that all the paperwork for the 2017 Bodywork Practitioner License for Ellen <br /> Demers Wakefield appears to be in order. <br /> Mallory Langler made a motion that the Board of Health approve the 2017 Bodywork Practitioner License for <br /> Ellen-Demers-Wakefield at Lyme Awareness-of-Cape Cod. Laurel Almquist seconded-the-motion.. The.motian. --- -__ <br /> was unanimously approved. <br /> 4. FY 2019 Board of Health Budget and Capital Improvement Program. <br /> Glen Harrington asked if the Board had any comments on the I Y 2019 Board of Health budget. Mr. Harrington <br /> commented that since there are new members on the Board he budgeted an extra amount within the <br /> Professional Development Account for training purposes. <br /> 2 <br />
