Town of Mashpee <br /> 4 }y7# A <br /> c /. 16 Great Neck Road North <br /> Mashpee, Massachusetts 02649 <br /> BOARD OF HEALTH - PUBLIC SESSION MEETING <br /> Mashpee Town Hall —Waquoit Meeting Room <br /> Minutes of the Meeting of November 30, 2017 <br /> Board Members Present: Also Present: <br /> Brian Baumgaertel, Chair Glen Harrington, Health Agent <br /> Laurel Almquist, Co-Chair <br /> Mallory Langler, Clerk <br /> Chair Brian Baumgaertel called the meeting to order at 7:00 PM. <br /> APPOINTMENTS <br /> 7:15 PM — "Show Cause" Septic Inspection Violation — 57 Camden Lane (Jason Haskell). <br /> Glen Harrington stated that the septic inspection performed at 57 Camden Lane on June 9, 2017, by Jason <br /> Haskell was reported as a "PASS" but should have been a "Needs Further Evaluation by the Local Approving <br /> Authority." Order letters were sent on September 25, 2017, and October 25, 2017, notifying the septic <br /> inspector of the deficiency and the pending "Show Cause" hearing. Telephone calls were made to the <br /> inspector's office that were ignored. The property transferred on July 14, 2017, with the existing deficiencies <br /> no well sample was taken and the risers were not brought up to grade. Mr. Harrington felt that since this was <br /> the first inspection Jason Haskell performed in Mashpee, Mr. Haskell was not familiar with the requirements. <br /> Since the property has transferred, the new owners are unaware of the two deficiencies. <br /> Mr. Harrington contacted All Clear Septic &Wastewater Services and reminded them of the "Show Cause" <br /> hearing for which written notification had been sent and asked if anyone would be present for the meeting. Mr. <br /> Harrington did inform them that fines may be issued and Mr. Haskell's license may be revoked, <br /> Mallory Langler made a motion that the Board of Health issue a $300.00 fine and revoke the Septic Inspector's <br /> permit, Mr. Jason Haskell, to inspect septic systems in Mashpee.for one year, or until the fine is paid and the <br /> septic inspector comes before the Board to have the permit re-issued. Also, NEIWPCC or the appropriate <br /> State of Massachusetts point of contact for the septic inspector's license is notified of his permit being revoked <br /> and the reason for the revocation. Laurel Almquist seconded the motion. The motion was unanimously <br /> approved. <br /> 7:20 PM —"Show Cause" Housing Code Violations— 62 Musket Lane (George Cummings). <br /> Glen Harrington stated a show-cause letter was sent to the owner of 62 Musket Lane to appear before the <br /> Board at the November 2, 2017, meeting. Both the First Class Mail and Certified Mail letters were returned. <br /> Therefore, the hearing was postponed so legal notice may be served. The letter was resent to another more <br /> current address for the November 30, 2017, meeting. The owner contacted the Board of Health, and the <br /> Assistant Health Agent inspected the dwelling and witnessed the two remaining violations were corrected. No <br /> further action is required. <br /> Mallory Langler made a motion that the Board of Health dismiss the orders of correction against Mr. George <br /> Cummings concerning 62 Musket Lane. Laurel Almquist seconded the motion. The motion was unanimously <br /> approved. <br /> 1 <br />
