• <br /> 5 <br /> OUTREACH COORDINATOR'S REPORT <br /> June 2010, <br /> HOME VISITS/APPOINTMENTS <br /> Twenty-three were made this past month, in addition to client contact by phone call. <br /> I assisted seniors experiencing a mental health crisis, i.e., significant depression, anxiety, <br /> domestic violence, etc. I worked collaboratively with the emt' , police, Human Services <br /> Director, etc, <br /> Several seniors I net with were experiencing social isolation, due to health issues, memory loss, <br /> no longer driving, and mental health concerns. Finding creative solutions that can b <br /> implemented continue to be a challenge. These eases are often time consuming, and goals, if <br /> achieved, are small. <br /> 1 continue to receive phone calls and meet with elders experiencing financial problems. I expect <br /> this to continue, especially as the state of Massachusetts is encouraging elders exhausting <br /> unemployment insurance benefits to contact heir senior center for help, <br /> VOLUNTEERS <br /> I continue to meet monthly with the telefriend, SHIT , friendly visitor assistant, grief support, <br /> birthday flower delivery, fuel assistance, and biography volunteers, to provide supervision and <br /> assign clients. <br /> I trained a new friendly visitor and a new driver. In spite of adding new drivers to our list, we <br /> are still in need of more drivers, as volunteer ride requests continue to increase. <br /> Volunteer Joan Pennell did an excellent job of putting together get well l bast ets that I will <br /> distribute to our clients when visiting them in the hospital or rehabilitation center. <br /> MEETINGS/CONTACTS <br /> I attended the CIRCA training for Council on Aging staff on leadership. <br /> I attended the ton's hoarding task force's monthly meeting. Currently the task force is working <br /> on developing a resource list.and a protocol for dealing with cases of boarding. <br /> Finally, Y met with Bereavement Coordinator Ann Geagan of Hospice and Palliative Care of <br /> Cape Cod, to discuss hove we can work together for Mashpee seniors. <br /> Respectfully Submitted, <br /> Lynne Waterman <br /> LF /Iv <br />
