w
"2-
y AppointTent w Dr. George Silve re; Medical Facility Plan Route 28.
611 r: I had originally come before ore i board for pemission to build n 11000 sq.ft b i g serer i d by aprivate
11. I was told that I could bui Id a 6000 sq ft bui 1dg w a private -wel 1 and if I warmed a larger bldg i would
have to go before DE E. This I did. I got a letter of support from Judy Mills, Mashpee Water comissioner and
from Tam Fudala, Town Planner. I have a copy of.bout letters w1w. DEQE sent an investigator to look at the
property and they essentially turned down my proposal. They said them wasn't enough land to do that, that l
would either have to get more land or get an easewnt from my abbutor to be able to put in a public non-
community wel i. What I'm before you for today is for perrmi ss ion to bu i 1 d the 6000 sq ft building which ,you
previously granted me permission to do, but that the 6000 sq ft building refer only to office space for
people to work in PLUS, an atrium lobbyof ca 1200 sq ft. The idea is that when town water eventual ly ccmes,
1 can add on to the other side of the lobby* The atrium will be used as lobby space only, not to see patients
in*
CFB.: It cages back to the starting key which is the nurber of owlets and the number of people occupying the
bldg. As long as ,you stay below the limits of 15 connections and 24 people, the open lobby isn't a problem.
The lobby will have no facilities or plumbing connections?
t
DR: That's correct.
CRC: Do you have an Eng i neer d raw i rig the Plans?
R; Walter Yarish and Cape & Islands
CRC: I don't have a problem w/what you're proposing- Do ,you EAM' -, # ,
FAVI: The BOH has already given him permission to build the 6000 sq ft bldg on the condition of limiting
outlets and occupants, My concern is water over the dart, I don t t know how you are going to control the
of patients you will be seeing w another DR. Y
DR: I'll have one other DR' two nurses-and,-,-two other pe p 1 e. T have 4-5 people in ray office at one t i .
The turnover is just like that. - - - - ° A. ,
GRC: We Meed the service in town and 1 want to do everything possible to help hire. Are you asking us to ask
DEE for you.
DR:. No. > E E has already said I can't 'put up a 1100 sq ft bldg. Mnasking you if the Town of Mashpee will
ive me per mi ss ion to put up a 6000 plus an 1200 sq ft lobby.
We gage a similar permission to P i rte Tree Corners,, which DEE is now questioning because of its s i e,
as long as you can prove to DEE, should they question you, that ,you are not exceeding the limits. i don't
see any problem granting the same type of request we granted thern., but its up to you to prove you are complying
with the limitations.
DR: How do I prove that. Are they gust going to walk in one day?
CRC;They could
CFB: We've cautioned builders/owners before about such limitations and these are recorded on the plans. The
usage pr j t ion indicates that them won't be a greater # of people us i rrt the b 1 d for-acre that 6 wth s
out of the year..
CRC: I th i nl I rade a mistake when I voted to OKthe BLDG at P i rte Tree. The size of it a 1 one worries me.
We did make it vent.clean to them what the limits were and we have documents to indicate they are aware
of these.
CFB: You have to go back toyour architect and tell him to be sure that no more than 15 connections will
be installed.
C: our concent is what ,your i wted i ate plans are, not what you plan to expand it to when pub l i c,water is
available.
EAM: Keep accurate records to protect yourself. Nor of Flo ees, # of patients, etc. There will be
occasions where if you're called out on an emergency there might be a backlog of patierts. but as long as
this is a rarty, thats OK.
CFB: You have to be over the limit of peop l e for 6 rrtths out of the ,year to be considered in violation.
DR: l " ve to l Iced to Delta Power about put i ng a pub 1 i c well on their property to sham w/me but they're
not interested. I have a lot of greenspace due to Delta Power's septic location and the road setback requir -
m rrts
GRC: You'd d better check w/the new zoning regs which are about to go irTto effect. You'd better file someth i n
fight away, So what do ,you want frram us'
R: A statement from you saying I can build a 6000 sq ft building for office space and a lobby of 1200 sq ft
not to be used for anything but a waiting area no water hook-ups.
CFB: We will word in such a way that you are providing a community service and that we are assured that
,you will exceed the limits imposed on you by a private well. Thea will be no more than 15 connections nor
more than 24 occupants, the size isn't that important* If ,you go w/the bigger bldg DEE would come in and
want you to seal off the additional space.
