MINUTES October 17, 1988 2 BOARD of HEALTH <br /> 9. Report, Quashnet Dalley to D.E. .E, Re.. Final assessment of soil con. <br /> tamination - Mr. McQuaid explained to the Board that the report re- <br /> commends that soils he removed on the mound and covered with plastic, <br /> soils below those removed be analyzed and if the analysis. is clean <br /> then no further excavation will be necessary. <br /> Mr. Costa suggested Mr. Greelish review the assessment and forward his <br /> comments and recommendations at the next regularly sch -du l ed meeting. <br /> 1r. Greelish was in ageeanoe. <br /> 10, Repair/upgrade Tavares for 'lot 19 Fells Circle - Mr. Costas stated <br /> this Board has received a memo from Ms. Graves which states the system <br /> was viewed and found -to he in failure, <br /> Mr. McQuaid id informed the Board that the original system was installed <br /> in 1968 with a 1 00 gallon tank and one -pit and explained they propose <br /> to discontinue the pit nd 'install two flow diffusers* <br /> Mr. costa stated the wetlands setb'ack was not delineated on plans, there- <br /> fore requested Nor. McQuaid to consult with conservation as to whether <br /> approval has -been obtained from them. <br /> s <br /> Mr, costa motioned -to continuo this repair/upgrade request pending the <br /> Conservation Comm ission`s recommendations and or~ approval , fir. Gree Iish <br /> seconded, all agreed. <br /> 1 '1 , ) , Pre-hearing plan review, Title V variance request (Ellis & Thulin) 763 <br /> -Fells Pond Road - Mr. McQuaid explained to the Board that -the appli- <br /> cant is requesting Variances from the sideline, well location and wet- <br /> lands setbacks to leaching facility. <br /> Mr. costa requested Mr. McQuaid to inform the applicants that they are <br /> required to consult with the conservation ommi.ssion anal may require <br /> approval from thea raor-Ao the issuance of a septic permit. <br /> Mr. Costa requested Mr. McQuaid to verify the list of .a.butters and schedule <br /> a pubic hearing date. He suggested the conservation Commission be noti- <br /> fied of the date for possible attendance or written recommendations. <br /> 1 . Pre -Hearing plan review, Title v' varianee request (Arrow Engineering , <br /> 49 captains Row - Mr. McQuaid i of or�med the B a r�d that the- applicant i s <br /> required to r qbe t' a. Variance from the Mashpee regulations only, however <br /> questions have been raised as to whether the gallonage in the leaching <br /> pit should be iner'eased or a reserve tank- installed due to its close <br /> proximity to Popponesset Bay. <br /> [fir. Costa stated the Board of Health had previously adopted regulations <br /> requiring 1 ,000 gallon leaching pit, the applicant is proposing 352 ,P. ... <br /> leaching pit which- will also require a Variance. <br /> Mr. Costa requested --.lir. - 1c Quay i d=to question the engineer as to whether <br /> revised plans c u l d be submitted showing a proposed 6x6 p-i t with a"bu l 1 <br /> run"' valve and a reserve area so that the systems can he alternated on <br /> d 0 an annual basis, if the engineer does not submit revised plans then a <br /> Variance will be required. <br />
