The <br /> rest of the specifics are that there will be no permits until the inspection is done <br /> with a floor plan submitted for review. They need to provide a contract with a <br /> licensed medical or biohazard waste company. provide a copy ofdisclosure <br /> statement that is to be given to the customers to advise of the risk of body art. That <br /> should be on the establishment's letterhead. The regulation states that the name of <br /> the establishment, address and phone number has to be on that disclosure <br /> statement. He feels it should be on the letterhead. <br /> Ms. warden asked if there was sometlhing in the regulation that states that the <br /> disclosure statement must be kept on file at the HH. <br /> Mr. Harrington confirmed that that was part of the regulation. He continued with <br /> the specifics that included the applicant provide a copy of the written instructions <br /> for aftercare. He doesn't have something like this as an example. He is hoping that <br /> they would have something like Haat from the state. He doesn't have it yet. Provide <br /> a copy of the client questionnaire form. That questionnaire form would have the <br /> past history ofthe client with a release statement signed by the client. This would <br /> include a copy of the.injury report forma. All those things should be in place before <br /> they open. Should the first guy they have be a hemophiliac and not tell them. They <br /> have an injury report form with all of the contacts. He'll make them aware that that <br /> is what he is looking for. Hopefully they will have all those iters the next time that <br /> they come before the board. <br /> Mr. Santos made a motion to approve the license of the practitioner on the <br /> stipulation that they verify some of his documentation.
