TOWN OF MASHPEE <br /> BOARD OF HEALTH <br /> June 27'", 2003 <br /> Meeting was called to order at 7:05 PM. <br /> Members present: L. Glenn Santos, Steven R. Ball, MaryRose Grady, Glen E. <br /> Harrington and Ms. Veronica Warden. <br /> THE TAPE IS NOT PART OF THE MEETING: IT IS FOR THE PURPOSE OF <br /> TRANSCRIPTION ONLY. <br /> Mr. Santos, Chairman <br /> 1. Sian Warrants. <br /> Warrants Signed. <br /> 2. Approve minutes of April Vl, 2002. <br /> Corrections were noted. Mr. Ball made a motion to approve the minutes. <br /> Mr. Santos seconded the motion. Motion passed. <br /> 3. Emergency Repair Certifications: <br /> 30 Starboard Drive <br /> Mr. Harrington indicated that the leaching pit was failed by inspection <br /> (Graci) and confirmed to have slime layer on sidewalls above the inlet <br /> with solids in the inlet pipe and holes filled with solids. The intent was to <br /> install two 500-gallon chambers with 4' of stone. <br /> The board members forwarded their signatures to the Emergency Repair <br /> Certificate. <br /> 42 Lowell Road <br /> Ms. Warden indicated that the leach pit was full into the risers. The intent <br /> is to install two 500-gallon dry wells with 4' of stone and a new D-box. <br /> They will remove the old pit. Local variances will be required for abutting <br /> wells and on-site to on-site. <br /> The board members forwarded their signatures to the Emergency Repair <br /> Certificate. Mr. Ball made a motion to grant the local variances from well <br /> to septic to abutting wells that are over 100' along with onsite to onsite. <br /> Ms. Grady seconded the motion. Motion approved. <br /> APPOINTMENT: The Phoenix Group 194 Fells Pond Road <br /> Mr. Grotzke wasn't present at 7:15 PM. The board stated that they would <br />
