r <br /> r - r <br /> TOWN OF MASHPEE <br /> BOARD OF HEALTH � PUBLIC MEETING <br /> Minutes of the Fleeting of Wednesday, lune 7,2006 4 <br /> Board Members Present: Board Members absent; Also Present: <br /> Glenn Santos Steven Ball Glen Harrington, Health Agent <br /> Lucy Burton Veronica Warden,Assistant Health Agent <br /> Vice Chairperson Lucy Burton called the meeting to order at 7:00 PK <br /> APPOINTMENTS <br /> Cape& Islands Engineering—29 Frog Pond close.. <br /> Delineation of the water main on the street is not shown on the plans presented by Cape &Islands. <br /> Glenn Santos moved that the BOH approve plans presented by Cape& Islands for the property at 29 Frog <br /> Pond Close, pending receipt of revised plans shoving the location of the water main on the street, and <br /> meeting all the Title V requirements. Lucy Burton seconded the motion. All approved. <br /> Shaw's s perfnar et—Consultant scope of�Services <br /> Dennis Kwider, Director of Quality Assurance for Star Market, and Mike Zajko, Store Manager, introduced <br /> themselves to the Board. Dennis Kwider stated that the consultant, Patron Associates, hired by Shaw's, <br /> will begin next week. <br /> Glenn Santos made a motion to approve the scope of services for Shaw's Supermarket, as submitted by <br /> Patron Associates. Lucy Burton seconded the motion. All approved. <br /> Bayview condos-381 Great oak Road South'-septic Vadance Request <br /> Frank Leo, Chairman of the Bayview Condominium Association, introduced himself to the BOH. <br /> The Bayview.Condominium leaching facility was found to be located underneath their paved tennis courts. <br /> The BOH septic inspection r gulaUon requires that risers be brought up to within six inches of grade. The <br /> leaching facility consists of long rows of four x four galleys, each with a twenty inch cover that could be <br /> raised if any portions of the covers are not underneath the playing surface of the tennis court. Glen <br /> Harrington stated that he asked Brad White, the septic inspector, if he could determine whether any portion <br /> of the covers were not under the tennis court playing surface. If the entire leaching Meld is underneath the <br /> surface of the tennis court, then a video camera could be utilized to inspect the Teaching field. <br /> Glenn Santos made a motion to continue the decision on the septic variance request by Bayview <br /> Condominium Association until the enact location of the covers of the leaching field can be determined. <br /> If any of the covers for the four trenches are found to be outside of the playing area of the tennis courts, <br /> those trenches would meet the BOH requirements for inspection. Those trenches that may fall 100% under <br /> the tennis court surface would be allowed to be inspected by camera. Lucy Murton seconded the motion. <br /> All approved. <br /> BSS Design—86 Summersea Road <br /> Glenn Santos made a motion to deny the request from 86 Surnmersea Road for a Nitrogen n Aggregation <br /> Plan, based on the fact that the proposed property is already being tied to another piece of property being <br /> used for the same purpose. Lucy Burton seconded the motion. All approved. <br />
