REQUEST FOR TITLE V VARIANCE 62 WATERLINE DRIVE <br /> Michael Grotzky, the designing engineer for this property, has left the country and is not available to issue a <br /> certificate of compliance for the installation at 62 Waterline Drive. The BOH Agent hes inspected the work <br /> and verified that the system was installed per plan. <br /> Glenn Santos made a motion to approve the Title V variance request for 62 Watedine Drive, without the <br /> designing engineer's Certificate of Compliance. Lucy Bu rton seconded the motion. All approved. <br /> 6. REQUEST FOI TITLE V&130H SEPTIC VARIANCES—26 LITTLE NECK LANE <br /> Glen Harrington stated that he has not had time to review the details of the request. <br /> Glenn Santos made a motion to continue discussion of the request for Title V and BOH septic variances at <br /> 26 Little Heck Lane to the next BOH meeting. Lucy Burton seconded the motion. All approved. <br /> DEFINITIVE SUBDIVISION PLAN Evim- MERCANTILE WAY(ROMAR DRIVE) <br /> Glenn Santos made a motion to approve the definitive subdivision plan for Mercantile Way(Romar Drive). <br /> Lucy Burton seconded the rnobon. All approved. <br /> . Food Establishment Permit Application—Subway Mashpee <br /> Veronica Warden n reported that there hes been a change of ownership in this establishment, with no other <br /> charges made. Carrie Tom iewicz is the new owner. <br /> Glenn Santos made a motion to approve the food establishment permit for Carrie Tom iewicz , new owner <br /> of the Subway Mashpee, pending proof of Sear Safe Certification. Lucy Burton seconded the motion. All <br /> approved. <br /> . Commercial Site Plan Title V Permit Application: .340-342 Main Street <br /> Glen Harrington reported that the plans include a retail showroom area, offices, and a warehouse. The <br /> will have a 1500 gallon tank with a point 5 Fast unit, going out to a Buil Run valve <br /> that goes out to a primary and reserve areas built out at the same bine. The flow is not over boo gallons <br /> per day. The entire property is in Zone 2. They will want the credits with the denitratri cation system for <br /> future additional units. <br /> Glenn Santos made a motion to approve the Commercial Site Plan and Tifle V permit application a <br /> subr ifted for 340-342 Mala Street In Mashpee, dated 5/31/06$ with the standard Commercial building <br /> restrictions applyiin , including no automotive repair, no storage of hazardous wastes, and the standard o <br /> in plane for the Fast System, and any change in ownership has to be approved by the BOH.prior to the <br /> chance in tenancy ownership. Lucy Burton seconded the motion. All approved. <br /> 10. Request for Food Establishment Permit—David Burton. <br /> The BOH received a request from David Burton for a food establishment permit for a hot dog cart. The <br /> discussion will be continued until the next BOH meeting when more information is available about the <br /> request, <br /> OLD U I E S <br /> Deer crossing Commercial Condominiums <br /> ms <br /> Patrick Brady,Attorney for the Deer Crossing Commercial Condominiums, introduced himself to the Board. <br /> 1!Illr. Brady requested the initial installation of the denitrification units to begin after Labor Day due to <br /> businesses being affected by summer tourists. New denitrification technology is proposed which will <br /> require new BoH approval. Bortolotti will be doing the work. <br /> Glenn Santos made a motion to continue discussion of the issue to the next meeting, on June 21, 200 6. <br /> Lucy Burton seconded the motion.
