Falmouth, MA 02536 Certified Mail #7006 2150 0001 0824 8688 <br /> RE: UIC Notification Letter for 131-Industrial Drive,Mashpee,MA <br /> Dear Mr. Kerrigan, <br /> On October 24, 2008, Glen Harrington, R.S., Health Agent for the Town of Mashpee <br /> and Bob Murphy of the Massachusetts Department of Environmental Protection (DEP) <br /> inspected the property known as Assessor's Map 88, Parcel 99, 131 Industrial Drive, <br /> Mashpee, MA. The property was inspected due to a complaint received by DEP. At that <br /> time, a floor drain was observed at the property. The floor drain appeared as a six-foot <br /> diameter catch basin with cast iron grate at grade in the western bay. <br /> Due to the presence of the floor drain, the discharge point is considered an <br /> underground injection control (UIC) system which is a violation of the Board of Health <br /> Toxic & Hazardous Materials Regulation Part XVII Section 1.00 —Floor Drains (enclosed). <br /> Please review the following options for your property. All options below include the proper <br /> "closure" of the UIC, according to 310 CMR 27.00 and the Board of Health regulation. <br /> As mandated under the Federal Safe Drinking Water Act, the state UIC regulations <br /> prohibit potentially polluting discharge to injection wells. Vehicle maintenance operations <br /> commonly use unauthorized injection wells, such as floor drains leading to a septic system, <br /> dy well, or oil/water separator which leads to any subsurface leaching structure. <br /> Under the State Plumbing Code (248 CMR 2.09 (1) (c) (3)), owners/operators of <br /> facilities with floor drains tied to injection wells (or discharging to any surface point) have <br /> three options: <br /> 1. Seal the floor drain. Contact you local plumbing inspector for the appropriate <br /> filing form. If choosing this option, all previous discharges to the drain must be <br /> eliminated at the source. For example, cars should no longer be washed and <br /> floors should no longer be hosed down. <br /> 2. Connect the floor drain to a holding tank. The tank will need DEP approval. The <br /> DEP approved two types of holding tanks for this waste: new installations and <br /> conversions of existing structures (e.g. oil/water separators). These tanks are for <br />
