tl <br /> `' <br /> non-hazardous industrial wastewater. If solvents, antifreeze, oil and other fluids <br /> are washed down the drain, the waste is likely to be hazardous. <br /> 3. Connect the floor drain to a municipal sewer system, if available. An oil/water <br /> separator is required to be installed under this option. This requires a permit from <br /> DEP and the Town of Barnstable Department of Public Works along with the <br /> sewer connection application. The amount of discharge shall not exceed ten parts <br /> per,million(10 ppm). <br /> CLOSURE OF THE UIC <br /> The"closure" of the UIC includes verifying the location and type of discharge <br /> facility, sampling/analyses of wastewater, sludge and soils, backfilling of discharge facility, <br /> and notification to proper authorities. A copy of DEP's recommended guidelines are <br /> available for your revie4 at http //www mass gov/dep/water/laws/uic.htm. <br /> In all cases, the owner must file a UIC NOTIFICATION FORM with DEP. <br /> You are directed to comply with the state's UNDERGROUND INJECTION <br /> CONTROL regulations(3 10 CMR 27.00) and the Board of Health regulation by informing 4 <br /> 4V <br /> this department in writing of your intentions within ten(10) days of receipt of this notice and j <br /> completing the work*ithin thirty(30) days. <br /> PER ORDER OF THE BOARD OF HEALTH <br /> Glen E.
