DRAFT 10/27/10 <br /> [S.C. 8/4/041 <br /> File: JLCD <br /> PROTOCOL FOR MANAGING STUDENTS WITH LIFE-THREATENING ALLERGIES <br /> When a student enters Mashpee Public Schools and is identified as <br /> having a diagnosis of a life-threatening allergy, the following <br /> plan of action will take place to provide a safe environment for <br /> the student to attend school . Each student will have individual <br /> modifications based on the severity of the allergic response as <br /> documented by the physician. <br /> RESPONSIBILITY OF THE SCHOOLS <br /> • School nurse documents severe allergic reaction history <br /> following discussion with parents. <br /> • The nurse and parent will develop an Individual Health Care <br /> Plan. <br /> • Appropriate staff members are informed of student' s <br /> allergies and Individual Health Care Plan. <br /> • Epipen: <br /> ➢ Appropriate documentation is on file containing a <br /> physician' s order, parental consent form, and <br /> medication administration plan which includes plan for <br /> field trips . The student' s photograph is attached to <br /> the medication form. <br /> ➢ Training regarding the use of the Epipen and allergic <br /> reactions will be conducted for staff members annually. <br /> ➢ The nurse maintains and periodically checks the Epipen <br /> for expiration date and notifies parent (s) when a <br /> replacement is needed, but it is the parent' s <br /> responsibility to ensure that the Epipen is current. <br /> ➢ If an allergic reaction occurs, the Epipen must be <br /> administered at once, EMS must be contacted, the <br /> student/individual must be transported to the hospital, <br /> and the parent must be notified. <br /> ➢ Appropriate dose of epinephrine is on hand at school <br />
