it <br /> Fire Station Buildffig Committee <br /> Minutes <br /> November 30, Zoo <br /> PROJECTOVERVIEW: <br /> In addition to the construction schedule lapse and time delays, safety <br /> issues and change orders were reviewed. It was noted that OSHA was on- <br /> site on November 9, 2000 and detected significant project safety issues. <br /> Additional safety issues continue to arise, flatters of concern include <br /> employees on-site without the use of safety lines and open holes left <br /> unattended with the absence of safety fencing. The Director of Public <br /> Works was also noted to be concerned with the issue relative to the setting <br /> of the tight tank. A 2 ' hole was noted to be unattended and without <br /> safety fencing. The liability issue of this-matter was discussed. <br /> Project change orders and associated liability of such was also deemed a <br /> priority of concern. It was noted that many of the change order directives <br /> are often modified and further delayed as a result of approval procedures <br /> and pending evaluations. <br /> Project errors and omissions were also discussed. Liability issues were <br /> also considered in addition to the apparatus equipment that would be <br /> housed outdoors during the winter months. <br /> In concern., the Committee agreed to consult Town Counsel with respect to <br /> this regard. The principals of The Wallace Floyd Design Croup and <br /> Standen Construction Company, Inc. will be requested to discuss the <br /> continuing project issues with the Building Committee in an effort to bring <br /> forth a resolution for substantial project completion. <br /> The Committee also agreed to post weekly on-site project meetings in the <br /> event of a quorum or if a necessitated vote of approval is warranted ire an <br /> effort to move the project forward in a timely fashion. <br /> Chane Order#11 - $1 J 16.42: <br /> To furnish and install rnesl�on first floor slab. SIG recommends <br /> approval. The installation of the mesh was not written in the <br /> specifications although this is noted to be that of standard construction <br /> practice. The building code requirement will be reviewed with the <br /> Building Inspector. It was noted that all construction should be in <br /> accordance with contract specifications and documentation. <br />
